The development of health care model for elderly people with hypertension through family and Community Participation in Nakhu Subdistrict, Nakhu district, Kalasin province

Purposes : To studied development of health care model for elderly people with hypertension through family and community participation in Nakhu subdistrict, Nakhu district, Kalasin province.
Study design : This research was action research
Materials and Methods : The target group is 1) 55 elderly people with high blood pressure in the community, 2) 70 relatives/caregivers of elderly people with high blood pressure in the community, and 3) elderly people with high blood pressure who are unable to blood pressure levels were controlled within normal limits for 70 people. Selected specific samples (Purposive sampling) used questionnaires and group discussions to collect data between October 2022 - June 2023. Data were analyzed using frequency, percentage, mean, standard deviation and Paired sample t- test.
Main findings : After the development, the sample group had a significantly higher mean score of knowledge about high blood pressure and health care according to the 3O2S guidelines in the community than before the development (p<.05), with the score The average score was 2.52 points (95%CI=1.67-3.33) and there was an average practice in taking care of the health of elderly people with high blood pressure in the community. Statistically significantly higher than before development (p<.05), with an average score increasing by 1.10 points (95%CI=1.01-1.17).
Conclusion and recommendations : The research resulted in improved health behaviors and clinical outcomes.
