บทคัดย่อ
บทนำ: จังหวัดสตูลมีผู้ป่วยโรคความดันโลหิตสูงทั้งหมด 30,008 ราย อัตราการควบคุมค่าความดันโลหิตไม่ได้สูงถึงร้อยละ 56.31 สำหรับอำเภอควนโดน จังหวัดสตูล ผู้ป่วยโรคความดันโลหิตทั้งหมด 1,059 ราย พบอัตราการควบคุมค่าความดันโลหิตไม่ได้ ร้อยละ 56.94 ผู้วิจัยจึงสนใจศึกษาปัจจัยที่มีความสัมพันธ์กับการควบคุมค่าความดันโลหิตไม่ได้ของผู้ป่วยโรคความดันโลหิตสูง อำเภอควนโดน จังหวัดสตูล
วิธีการศึกษา: การวิจัยแบบ Cross-sectional study โดยทำการศึกษาในกลุ่มผู้ป่วยโรคความดันโลหิตสูง จำนวน220 คนที่มีอายุ 18 ปีขึ้นไป ในตำบลควนสตอ อำเภอควนโดน จังหวัดสตูล สุ่มตัวอย่างแบบมีระบบ เก็บรวบรวมข้อมูลด้วยแบบสัมภาษณ์ซึ่งประกอบไปด้วยข้อมูลส่วนบุคคล พฤติกรรมสุขภาพ และผลการตรวจทางห้องปฏิบัติการ ค่าความตรงเชิงเนื้อหา IOC เท่ากับ 0.95 สถิติที่ใช้ ได้แก่ การแจกแจงความถี่ ค่าร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน มัธยฐาน พิสัยควอไทล์ และ Logistic regression
ผลการศึกษา: พบปัจจัยที่มีความสัมพันธ์ต่อการควบคุมค่าความดันโลหิตไม่ได้ของผู้ป่วยโรคความดันโลหิตสูง ได้แก่ เส้นรอบเอว (p-value = 0.005) ระยะเวลาที่ถูกวินิจฉัยเป็นโรคความดันโลหิตสูง (p-value = 0.028) การได้มาของอาหาร (วัตถุดิบ) (p-value = 0.049) ค่าความดันโลหิต Systolic (p-value <0.001) ค่าความดันโลหิต Diastolic (p-value = 0.046) ค่า Total cholesterol (p-value = 0.002) ค่า LDL (p-value = 0.024, 0.002) ค่า eGFR (p-value = 0.048)
สรุปผล: จากการวิจัยครั้งนี้ค่าระดับไขมัน Cholesterol และ LDLเป็นปัจจัยสำคัญดังนั้นควรมีการพัฒนารูปแบบส่งเสริมสุขภาพผู้ป่วยโรคความดันโลหิตสูง โดยเน้นเรื่องการบริโภคอาหาร และคัดกรองภาวะแทรกซ้อนของโรคเป็นประจำ อันจะนำมาซึ่งคุณภาพชีวิตที่ดีของผู้ป่วยต่อไป
