บทคัดย่อ
บทนำ:การวิจัยและพัฒนามีวัตถุประสงค์เพื่อศึกษาสถานการณ์ การพัฒนารูปแบบ และผลของการใช้รูปแบบการเข้าถึงระบบบริการทางการแพทย์ของผู้ป่วยโรคหลอดเลือดสมองเฉียบพลัน ชุมชนบ้านเหนือฟ้า อำเภอพิปูน จังหวัดนครศรีธรรมราช
วิธีการศึกษา:กลุ่มตัวอย่างจำนวน 20 คน ประกอบด้วย ผู้ป่วยกลุ่มเสี่ยงต่อการเกิดโรคหลอดเลือดสมองเฉียบพลันหรือผู้ดูแลในครอบครัวจำนวน 10 คนและผู้ดูแลในชุมชนจำนวน 10 คน ด้วยการคัดเลือกแบบเฉพาะเจาะจง เครื่องมือที่ใช้ประกอบด้วย 1) รูปแบบการเข้าถึงระบบบริการทางการแพทย์ ของผู้ป่วยโรคหลอดเลือดสมองเฉียบพลันที่พัฒนาขึ้น 2) แบบทดสอบความรู้และแบบวัดความพึงพอใจของผู้ป่วยหรือผู้ดูแลและผู้ดูแลในชุมชน 3)แบบบันทึกระยะเวลาการเข้าถึงระบบ Stroke Fast Tract ในสถานการณ์จำลอง สถิติที่ใช้ประกอบด้วย สถิติเชิงพรรณนาคือ จำนวน ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมาน คือ Wilcoxon signed rank test
ผลการศึกษา:พบว่า1)รูปแบบการเข้าถึงระบบบริการทางการแพทย์ของผู้ป่วยโรคหลอดเลือดสมองเฉียบพลัน ชุมชนบ้านเหนือฟ้า อำเภอพิปูน จังหวัดนครศรีธรรมราช ประกอบด้วย ก) การระบุตำแหน่งบ้านหรือชุมชน (GIS/location) ข) การมีระบบผู้ดูแลในชุมชน (Care giver) ค) การรับรู้สัญญาณเตือนของโรค (warning signs) ง) การมีระบบขอคำปรึกษาแบบออนไลน์ (Application/ Telemedicine) และ จ) การนำส่งโรงพยาบาล ที่รวดเร็ว (Transport) 2) ความรู้และความพึงพอใจของผู้ป่วยหรือผู้ดูแลในครอบครัวและผู้ดูแลในชุมชน ก่อนและหลังการใช้รูปแบบการเข้าถึงระบบบริการทางการแพทย์ของผู้ป่วยโรคหลอดเลือดสมองเฉียบพลัน แตกต่างอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 และ 3) ระยะเวลาการเข้าถึงระบบ Stroke Fast Tract ในสถานการณ์จำลอง 38 นาที
สรุปผล:จากการศึกษาพบว่าผู้ป่วยหรือผู้ดูแลในครอบครัวและผู้ดูแลในชุมชนมีความพึงพอใจการใช้รูปแบบการเข้าถึงระบบบริการทางการแพทย์ของผู้ป่วยโรคหลอดเลือดสมองเฉียบพลัน ดังนั้นควรนำรูปแบบไปใช้สำหรับวางแผนการดูแลผู้ป่วยในพื้นที่อื่นต่อไป
