The Development of Care Guideline for Patients with Alcohol Withdrawal in the Case of Hospitalization to the Community Countinuously Yangsisurat District Mahasarakham Province

Abstract

This action research aimed to develop and evaluate of care guidelines for patients with alcohol withdrawal in the case of hospitalization to the community continuously. The groups involved in development of care guidelines consisting of octors, nurses, pharmacists, mental health nurses,addiction treatment nurse, representatives of patients and their relatives total Thirty-five, and a group of 210 patients with alcohol withdrawal syndrome. The research tools were the CIWA-Ar. Risk report, in-depth interview form, observation form, and patient medical records. Analyses of data using descriptive statistics and qualitative data. Results found that Appropriate forms of care for patients with alcohol withdrawal syndrome were: 1) Developing personnel knowledge about CIWA-Ar. assessment skills 2) Developing guidelines for screening and setting guidelines for care according to their level of severity 3) Enhance knowledge of the main caregivers in the family. 4) Develop a system for referring information to alcohol cessation clinics and in the community. 5) Together with the alcohol cessation clinic, design continuation of community care form of mindfulness therapy and family therapy. By a multi-disciplinary team through Line, a group, and a phone call per person to the alcohol withdrawal clinic. Evaluation found that 75,95,100 % of patients were screened and monitored for Alcohol Withdrawal Conditions, 18,13.33,5.30% had complications 8.6,8.3,4.0% of re-admit illnesses repeated within ≤ 4 months entered the alcohol cessation clinic and received continuous care, drinking decreased in all cases and were able to quit alcohol 19.05% Patients with withdrawal symptoms are frequently complications.The development of guidelines for inpatient has resulted in patients is safe and can reduce the complications that will occur. including reducing sleep time Information was sent to receive treatment in the alcohol cessation clinic and referral to the community for continuation of care.
