การวิจัยเชิงคุณภาพแบบบรรยายนี้ มีวัตถุประสงค์เพื่อศึกษา 1) กระบวนการในการดำเนินงานดูแลผู้สูงอายุในชุมชน
ของพยาบาลชุมชนที่ปฏิบัติงานในโรงพยาบาลส่งเสริมสุขภาพตำบล 2) ปัญหาอุปสรรค และแนวทางการพัฒนาระบบบริการการดูแลผู้สูงอายุในชุมชน โดยใช้กรอบแนวคิดเชิงระบบของโดนาบีเดียน ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพที่ปฏิบัติงานในโรงพยาบาลส่งเสริมสุขภาพตำบล จำนวน 24 คน เก็บรวบรวมข้อมูลโดยแบบสอบถามข้อมูลส่วนบุคคล และ การสนทนากลุ่ม วิเคราะห์ข้อมูลส่วนบุคคลโดยใช้สถิติบรรยาย และข้อมูลเชิงคุณภาพใช้การวิเคราะห์เนื้อหา
ผลการวิจัย พบว่า พยาบาลชุมชนรับรู้นโยบายการดำเนินงานด้านผู้สูงอายุจากสำนักงานสาธารณสุขอำเภอโดยตรง
และผา่ นทางผูอ้ ำนวยการโรงพยาบาลสง่ เสริมสุขภาพตำบล หลังจากรับทราบนโยบาย พยาบาลชุมชนมีกระบวนการดำเนินงานดูแลผู้สูงอายุในชุมชน 5 ขั้นตอน ได้แก่ 1) การศึกษาและทำความเข้าใจกับนโยบาย แนวทางปฏิบัติ 2) การสร้างความตระหนักให้เห็นถึงความสำคัญของการดำเนินงาน 3) การพัฒนาศักยภาพในการดำเนินงาน 4) การกำหนดเป็นแผนงานประจำปี และ 5) กาประสานความร่วมมือกับภาคีเครือข่าย ปัญหาอุปสรรคที่พบ ได้แก่ อัตรากำลังที่ไม่เพียงพอ ขาดการพัฒนาศักยภาพบุคคล วัสดุ อุปกรณ์และงบประมาณไม่เพียงพอ และ ระบบการประสานงานไม่ชัดเจน โดยแนวทางในการพัฒนาระบบ ได้แก่ การทบทวนและปรับอัตรากำลังให้เหมาะสม การพัฒนาศักยภาพ การสนับสนุนงบประมาณให้เพียงพอและการพัฒนาระบบประสานงานระหว่างเครือข่ายให้มีประสิทธิภาพเพิ่มขึ้น
การศึกษาครั้งนี้ชี้ใหเ้ ห็นถึงความสำคัญในการพัฒนาโครงสรา้ งเพื่อใหเ้ กิดกระบวนการในการดูแลผูสู้งอายุในชุมชนที่มี
ประสิทธิภาพ ได้แก่ การมีนโยบายและตัวชี้วัดที่ชัดเจนในการดำเนินงาน การจัดสรรอัตรากำลังคนและภาระงานให้เหมาะสมการพัฒนาศักยภาพของบุคคลให้มีความสามารถตามงาน รวมทั้งการสนับสนุนงบประมาณ วัสดุ อุปกรณ์ ที่เพียงพอจะส่งผลให้การดูแลผู้สูงอายุในชุมชน มีความครอบคลุม และมีประสิทธิภาพมากยิ่งขึ้น เพื่อคุณภาพชีวิตที่ดีของผู้สูงอายุ

Abstract
This descriptive qualitative researchaimed to describe 1) the process of care delivery for older adults in communities by community health nurses working at sub- district health promoting hospitals, 2) problems, obstacles,and approaches in developing care delivery process for older adults in the communities. Donabedian’s system theory was used as the conceptual framework. Key informantsincluded 24 community health nurses working in
sub- district healthpromoting hospitals. Data were collected by using demographic questionnaire and focus group discussion. The data were analyzed using descriptive statistics, and content analysis. Results revealed that community health nurses were informed of policy fromthe district health office directly and through the director of sub- district health promoting hospital. Process of care delivery for older adults in communities by Community Health Nurses consisted of 5 steps.1) Finding information, 2) Raising awareness, 3) Capacity building, 4) Stating within the year plan and 5) Coordination and collaboration. Obstacles included inadequate manpower, lack of capacity building, inadequate budgets and equipment, and unclear coordinating systems. Approaches to system development included review for appropriate manpower, capacity building, adequate budgetary support, and development of more effective coordinating systems among networks. This study suggests the importance of developing structure to achieve effective care process for community dwelling older adults which included obvious policy and indicators, allocation of manpower and workload optimization,

capacity building, adequate budget and equipment. These will contribute to more coverage and effective care of community dwelling older adults, resulting in increased quality of life.
