บทคัดย่อ

การวิจัยครั้งนี้เป็นการวิจัยเชิงบรรยาย เพื่อศึกษาองค์ประกอบสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงาน
การดูแลผูป้ ว่ ยโรคหัวใจ หลอดเลือดและทรวงอก คณะแพทยศาสตร ์ มหาวิทยาลัยขอนแกน่ กลุม่ ตัวอยา่ งคือพยาบาลวิชาชีพที่สำเร็จการศึกษาระดับปริญญาตรี หรือสูงกวา่ ปริญญาตรี ที่ปฏิบัติงาน การดูแลผูป้ ว่ ยโรคหัวใจ หลอดเลือดและทรวงอกในศูนย์หัวใจสิริกิติ์ภาคตะวันออกเฉียงเหนือ และโรงพยาบาลศรีนครินทร์ คณะแพทยศาสตร์
มหาวิทยาลัยขอนแก่น ปฏิบัติงานตั้งแต่ 1 ปีขึ้นไป โดยรวมเฉพาะหัวหน้าหอผู้ป่วยที่ยังคงปฏิบัติงานในเวรบ่าย
และเวรดึก จำนวน 350 คน เก็บข้อมูล โดยใช้แบบสอบถามความคิดเห็นเกี่ยวกับระดับความสำคัญของสมรรถนะ

ขอนแก่น หาค่าความเที่ยงโดยใช้สูตรสัมประสิทธิ์แอลฟาของครอนบาค ได้ค่าความเที่ยง 0.97 วิเคราะห์ข้อมูลโดย
ใช้วิธีวิเคราะห์องค์ประกอบหลัก (Principal Component Analysis) ผลการวิจัยพบวา่ องคป์ ระกอบสมรรถนะของพยาบาลวิชาชีพที่ปฏิบัติงาน การดูแลผูป้ ว่ ยโรคหัวใจ หลอดเลือดและทรวงอก คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น มี 13 องค์ประกอบ โดยจัดกลุ่มสมรรถนะเป็น 3 กลุ่ม คือ กลุ่มที่ 1 สมรรถนะพื้นฐานวิชาชีพการพยาบาล ประกอบด้วย องค์ประกอบ 5 ด้าน ได้แก่ 1) ด้านการสื่อสารภาษา ต่างประเทศ 2) ด้านปฏิบัติการพยาบาลทั่วไป 3) ด้านการเสริมสร้างพลังอำนาจด้านสุขภาพ 4) ด้านการใช้เทคโนโลยี สารสนเทศ และ 5) ด้านการพยาบาลข้ามวัฒนธรรม กลุ่มที่ 2 สมรรถนะเฉพาะสาขา ประกอบด้วย องค์ประกอบ 4 ด้าน ได้แก่ 1) ด้านการปฏิบัติการเฉพาะทางผู้ป่วยโรคหัวใจ หลอดเลือด และทรวงอก 2) ด้านการบริหารจัดการยา ที่ให้กับผู้ป่วยโรคหัวใจ หลอดเลือด และทรวงอก 3) ด้านการช่วยฟื้นคืนชีพขั้นต้นและขั้นสูง และ 4) ด้านการช่วย แพทย์ทำหัตถการ ทางโรคหัวใจ หลอดเลือด และทรวงอก กลุ่มที่ 3 สมรรถนะการจัดการ ประกอบด้วย องคป์ ระกอบ 4 ดา้ น ไดแ้ ก  1) ดา้ นภาวะผูน้ ำยุคใหม  2) ดา้ นความรูท้ างกฎหมาย จริยธรรมและจรรยาบรรณวิชาชีพการพยาบาล 3) ด้านพัฒนาวิชาชีพพยาบาล และ 4) ด้านวิชาการ วิจัยและสารสนเทศ องค์ประกอบเหล่านี้ สามารถอธิบายความแปรปรวนรวมได้ร้อยละ 78.2

The objectives of this descriptive research were to investigate the components of professional nurse competencies for Cardiovascular and Thoracic care, Faculty of Medicine, Khon Kaen University. The study sample consisted of 350 cases, including nurses who have Bachelor’s degree, equivalent certificate, higher degree of education, registered as first class professional nurses and having been working for cardiovascular and thoracic care in faculty of medicine, Khon Kaen University for at least one year and head nurse who having been working on duty except supervisor nurse and director of nursing department. The researcher developed the 2 part of questionnaire instrument. A questionnaire part 1 was
used to determine demographic data and part 2 was the cardiovascular and thoracic care nurse competencies. The validity of the content was approved by five experts and the reliability was analyzed by Cronbach’s Alpha Coefficients which was 0.97. The components of the instrument were analyzed by Factor Analysis. The research results were 13 components of cardiovascular and thoracic care nurse competencies emerged with eigen value 1 and accounted for total 78.2 % of variance. The 13 factors were classified
into 3 groups: Group 1 Basic nursing practice component: including 5 competencies: 1) Foreign language communication 2) General nursing practices 3) Health empowerment. 4) Information technology and 5)Transcultural nursing. Group 2 The specialty nursing component: including 4 competencies:1) Cardiovascular and Thoracic nursing practices. 2) Drug administration for Cardio vascular and Thoracic patient. 3) Basic and advance Cardio Pulmonary resuscitation. and 4) Assist the Physician in Cardio vascular and Thoracic operation. Group 3 Management component: Including 4 competencies 1) Transformational leadership 2) Law and Nursing ethics 3) Nursing development and 4) Knowledge,
