ประสบการณ์ของครอบครัวในการตัดสินใจถอดท่อช่วยหายใจผู้ป่วยระยะท้ายในหอผู้ป่วยประคับประคอง ศูนย์การุณรักษ์ โรงพยาบาลศรีนครินทร์: การวิจัยเชิงคุณภาพ

การยุติการใช้เครื่องช่วยหายใจในกลุ่มผู้ป่วยระยะสุดท้ายเป็นประเด็นที่ละเอียดอ่อนและส่งผลกระทบต่อจิตใจของญาติอย่างรุนแรง งานวิจัยเชิงคุณภาพนี้มุ่งทำความเข้าใจประสบการณ์ของสมาชิกครอบครัวในกระบวนการตัดสินใจและสภาพการณ์ที่เกิดขึ้นภายในหอผู้ป่วยประคับประคอง โดยใช้ระเบียบวิธีวิจัยเชิงปรากฏการณ์วิทยา คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจงจากสมาชิกครอบครัวที่มีส่วนร่วมในการตัดสินใจถอดท่อช่วยหายใจและได้รับการดูแลแบบประคับประคองในโรงพยาบาลจำนวน 10 ราย เก็บข้อมูลด้วยการสัมภาษณ์เชิงลึกจนกว่าข้อมูลจะอิ่มตัว สัมภาษณ์เชิงลึกกับผู้ดูแลหลัก 10 ราย และวิเคราะห์แก่นแท้ของประสบการณ์ด้วยแนวคิดของโคไลซ์ซี่ ทั้งนี้ได้ตรวจสอบความน่าเชื่อถือของข้อมูลตามเกณฑ์ของ Lincoln และ Guba ครอบคลุมความน่าเชื่อถือ ความสามารถในการถ่ายโอน ความเชื่อมั่นได้ และความยืนยันได้ โดยใช้วิธีการตรวจสอบข้อมูลสามเส้า การยืนยันผลกับผู้ให้ข้อมูล และการทบทวนโดยผู้เชี่ยวชาญ
ผลการวิจัย พบว่า ประสบการณ์ของครอบครัวแบ่งออกเป็น 5 ช่วงสำคัญ ได้แก่ 1) ระยะเผชิญวิกฤต: แม้จะตระหนกและรู้สึกไร้อำนาจต่อรองแต่ยังฝากความหวังไว้กับการรักษา 2) ระยะเผชิญความจริง: เมื่อเห็นความลำบากของผู้ป่วยผ่านอุปกรณ์สายระโยงระยางทำให้เกิดความทุกข์ใจ 3) ระยะหาทางออก: ความเชื่อมั่นในการตัดสินใจเกิดขึ้นเมื่อได้รับข้อมูลที่โปร่งใสและแรงสนับสนุนจากทีมสุขภาพ 4) ระยะสิ้นสุดการยื้อ: ความสงบในช่วงลมหายใจสุดท้ายของผู้ป่วยช่วยเยียวยาจิตใจคนข้างหลังและ 5) ระยะปรับตัว: การดูแลที่มีคุณภาพช่วยให้ข้ามผ่านความเศร้าโศกได้อย่างมั่นคง การสื่อสารที่มีประสิทธิภาพและการให้ครอบครัวมีส่วนร่วมอย่างแท้จริงโดยทีมประคับประคอง คือกุญแจสำคัญที่ช่วยสร้างประสบการณ์เชิงบวกและลดบาดแผล ทางใจหลังการสูญเสีย
