การพัฒนาบทบาทพยาบาลเรือนจำในการดูแลสุขภาพจิตผู้ต้องขัง

ปัญหาสุขภาพจิตและความผิดปกติทางจิตของผู้ต้องขังทั่วโลกมีแนวโน้มสูงขึ้น ในขณะที่อัตราการเข้าถึงระบบบริการสุขภาพจิตของผู้ต้องขังทั้งประเทศไทยและต่างประเทศยังต่ำเมื่อเทียบกับจำนวนผู้ต้องขังที่มีอยู่ เนื่องจากการขาดแคลนทีมบุคลากรวิชาชีพ เครื่องมือประเมินทางจิตเวช กระบวนการบำบัดรักษาและปัญหาการส่งต่อที่ซับซ้อน ตามมาตรฐานสากลกำหนดให้ทุก ๆ เรือนจำ จะต้องมีบุคลากรทางการแพทย์หรือพยาบาลวิชาชีพอย่างน้อยหนึ่งคนเพื่อให้บริการสุขภาพผู้ต้องขังแบบองค์รวมทั้งในด้านส่งเสริม ป้องกัน บำบัดรักษาและฟื้นฟูสมรรถภาพ บทความนี้มีวัตถุประสงค์เพื่อพัฒนาบทบาทของพยาบาลเรือนจำใน การดูแลสุขภาพจิตของผู้ต้องขัง โดยการเน้นทบทวนวรรณกรรมเกี่ยวกับระบบบริการสุขภาพจิตในเรือนจำและบทบาทของพยาบาลในเรือนจำทั้งในประเทศไทยและต่างประเทศ พยาบาลในเรือนจำเป็นบุคคลที่มีบทบาทสำคัญในการให้บริการสุขภาพจิตแก่ผู้ต้องขังในเรือนจำ ตั้งแต่การประเมินคัดกรองผู้ต้องขังแรกรับทุกรายหลังเข้าสู่เรือนจำ ให้การบำบัดรักษาด้านสุขภาพจิตและจิตเวช การติดตามการรักษาอย่างต่อเนื่อง การให้บริการผ่านระบบบริการสุขภาพจิตทางไกล การจัดกิจกรรมเตรียมความพร้อมผู้ต้องขังก่อนจำหน่ายออกจากเรือนจำ รวมทั้งการส่งต่อเพื่อการดูแลต่อเนื่องภายหลังผู้ต้องขังพ้นโทษกลับสู่ชุมชน ดังนั้นหากพยาบาลมีแนวทางในการให้บริการด้านสุขภาพจิตและจิตเวชสำหรับผู้ต้องขัง ก็จะช่วยให้ผู้ต้องขังที่มีปัญหาสุขภาพจิตได้เข้าถึงบริการสุขภาพจิตและจิตเวชมีความพร้อมในการกลับไปใช้ชีวิตตามนโยบายคืนคนดีสู่สังคม ป้องกันการกระทำผิดซ้ำและการกลับมาป่วยซ้ำ
