Nurse’s role in discharge planning for patients with acute coronary syndrome: a case study

Abstract

Abstract

            Discharge planning is crucial in the patients with acute coronary syndrome as it prepares for the transition from critical illness to self-care at home. Nurses play an important role in helping patients to care for and manage their health when discharged from the hospital. An effective discharge planning encourages the patients to transition from the hospital to home. This article presents discharge planning based on Transition theory and uses evidence-based practice in the nursing care plans for discharge patients with acute coronary syndrome.

This nursing care plan for discharge planning consists of 1) Discharge readiness assessment 2) Preparing for discharge planning to minimize the risk factor and 3) Individual consultation, practicing necessary skills, and follow-up evaluation in the transitional phase of the patient. In this article, a case study is analyzed to guide nursing practice. Therefore, the discharge planning can help patients take action to prevent the recurrence of the disease.
