การจัดการพยาบาลรายกรณีสำหรับบุคคลที่ต้องการบำบัดสุราในชุมชน Nursing Case management for person with alcoholic therapy In community

บทคัดย่อ

Abstract

This action research aimed to develop a case management program for individuals receiving treatment for alcohol dependence at Sumsoong Hospital.  A purposive sampling was done to select twelve individuals with alcohol dependence who agreed to participate in the program and seven relatives or caregivers of these individuals. In addition, twelve interdisciplinary team members were invited to join the project. Adopting a case management framework, this study began with a situational analysis of existing alcoholism treatment services and found that the patient care process and the alcohol treatment clinic’s care plan were not comprehensive.  Interdisciplinary team members lacked necessary competencies and did not collaborate with other related services.  There was no space designated for the therapeutic activities at the clinic and no in-patient ward assigned to admit patients during their withdrawal stage.  In addition, the treatment recipients did not continue throughout the program and had relapsing drinking episodes. Subsequently, they developed medical problems and accidents.

As a result, this study developed 1) a care plan for individuals during withdrawal stage and 2) a case management program by an interdisciplinary health care team for alcohol treatment in the community.  Registered nurses acted as a case manager who provided counseling, coordinated care, and identified community resources so the recipients could receive a continuing comprehensive treatment with medications and psycho-social-spiritual care.

The community alcohol treatment program was found to be effective, simple and beneficial to the program recipients.  The interdisciplinary team had a guideline that ensured treatment continuity.  The nurses felt more confident in caring for alcohol dependent individuals.  The program recipients received a punctual and comprehensive care with more safety.  In addition, they received home visits twice a month for 3 months.  Both the program recipients and their caregivers were satisfied with the services provided.  Lastly, the interdisciplinary team members were highly satisfied with this case management program   for individuals with alcohol dependence.

การวิจัยเชิงปฏิบัติการนี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบการจัดการรายกรณีสำหรับบุคคลที่ต้องการบำบัดสุราในชุมชน เข้ารับการรักษาในโรงพยาบาลซำสูง กลุ่มตัวอย่างคัดเลือกแบบเจาะจง คือผู้ที่ดื่มสุราแบบติด และผู้ที่ดื่มสุราแบบอันตรายที่ยินดีเข้ารับการบำบัด จำนวน 12 คน และญาติหรือผู้ดูแล 7 คน ทีมสหวิชาชีพ 12 คน โดยใช้แนวคิดการจัดการรายกรณี เริ่มจากการวิเคราะห์สถานการณ์ในการดูแลผู้ต้องการบำบัดสุรา พบว่า กระบวนการดูแลผู้ป่วยและแผนการดูแลทางคลินิกยังไม่ครอบคลุม บุคลากรทีมสหวิชาชีพมีความเชี่ยวชาญจำกัด การประสานการทำงานไม่ต่อเนื่อง ด้านสิ่งแวดล้อมยังไม่มีห้องเป็นสัดส่วนเฉพาะในการทำกิจกรรม ไม่มีหอผู้ป่วยเฉพาะรองรับผู้ป่วยที่เกิดอาการขาดสุรา ด้านผู้ป่วย พบว่า มารักษาไม่ต่อเนื่อง หยุดการบำบัดกลางครัน มีการกลับไปดื่มซ้ำ และมีโรคร่วมทางกายเกิดขึ้น ได้แก่ โรคทางอายุรกรรม อุบัติเหตุต่างๆ การพัฒนาได้ 1) แผนการดูแลผู้ต้องการบำบัดสุราในระยะถอนพิษ 2) แผนการดูแลผู้ต้องการบำบัดสุราแบบผู้ป่วยนอกในชุมชน โดยทีมสหวิชาชีพ และพยาบาลเป็นผู้จัดการให้คำปรึกษาชี้แนะ ติดตาม ประสานการดูแล หาแหล่งสนับสนุนในชุมชน ให้ผู้ต้องการบำบัดได้รับการรักษาตามแผนการดูแล ทั้งรักษาด้วยยา บำบัดทางจิตสังคมอย่างต่อเนื่องครอบคลุมทั้งกาย ใจ จิตวิญญาณ นอกเหนือจากนั้นยังพบว่า แผนการดูแลสามารถปฏิบัติได้จริง ใช้ง่ายไม่ยุ่งยาก เกิดประโยชน์ต่อผู้ต้องการบำบัด มีการทำงานร่วมกันเป็นทีมสหวิชาชีพในทิศทางเดียวกัน มีขั้นตอนต่อเนื่อง พยาบาลเกิดความมั่นใจในการดูแลผู้ต้องการบำบัดสุรา ผู้ต้องการบำบัดได้รับการดูแลตามแผน อย่างทันท่วงที ปลอดภัย และผู้ต้องการบำบัดสุราได้รับการติดตามเยี่ยม เดือนละ 2 ครั้ง เป็นเวลา 3 เดือน ผู้ต้องการบำบัดสุราและญาติมีความพึงพอใจต่อบริการที่ได้รับ ทีมสหวิชาชีพมีความพึงพอใจต่อการพัฒนารูปแบบในครั้งนี้
