ความสัมพันธ์ระหว่างปัจจัยคัดสรรกับการมีวินัยในการรับประทานยาต้านการแข็งตัวของเลือด RELATIONSHIP BETWEEN SELECTED FACTORS AND ORAL ANTICOAGULANT ADHERENCE

บทคัดย่อ

ABSTRACT

This descriptive research aimed to investigate anticoagulant adherence and relationship between selected factors and anticoagulant adherence among patients underwent mechanical heart valve replacement. Selected factors in this study included knowledge regarding oral anticoagulant use; personal factors including age, income, and duration of anticoagulation use; and health belief factors according to health belief model [HBM] (Becker, 1975) including perceived risk, perceived severity, perceived benefits, perceived barriers, and motivation related to oral anticoagulant use. One hundred and fifty patients who had heart valve replacement and took oral anticoagulants were selected purposively from patients who received care at the outpatient clinic of the Queen Sirikit Heart center of the Northeast. Data collection was conducted during January-March  2013 using newly developed questionnaires. Four questionnaires were developed based on HBM and review of relevant literature including demographic data questionnaire, knowledge of oral anticoagulant use questionnaire, HBM-based oral anticoagulation perceptions questionnaire, and oral anticoagulation adherence questionnaire. All questionnaires were evaluated for content validity by a panel of experts and found to have an average CVI of 0.83. The questionnaires were pilot tested for reliability and found to have an average alpha Cronbach coefficient of 0.85. Data were analyzed by descriptive statistics and Mann-Whitney U Test.

Study results revealed that 52.7% of the study sample had high level of anticoagulant adherence whereas 47.3% were found to have moderate adherence. It was found in the group with moderate adherence that perceived barriers of oral anticoagulant use had moderately negative relationship with medication adherence. Relationships between other selected factors and anticoagulant adherence were not found. In the group with high adherence, no significant relationships were found between any selected factors and anticoagulant adherence.

การวิจัยนี้เป็นการวิจัยเชิงบรรยาย มีวัตถุประสงค์เพื่อศึกษาวินัยในการรับประทานยาและความสัมพันธ์ระหว่างปัจจัยคัดสรรกับวินัยในการรับประทานยาต้านการแข็งตัวของเลือดในผู้ป่วยหลังผ่าตัดเปลี่ยนลิ้นหัวใจเทียม ปัจจัยคัดสรรที่ศึกษา ประกอบด้วย ความรู้ในการรับประทานยา ปัจจัยส่วนบุคคล ได้แก่ อายุ รายได้ ระยะเวลาในการรับประทานยา และปัจจัยตามแบบแผนความเชื่อด้านสุขภาพ ได้แก่ การรับรู้โอกาสเสี่ยง การรับรู้ความรุนแรง การรับรู้ประโยชน์ การรับรู้อุปสรรค จากการรับประทานยา และแรงจูงใจในการรับประทานยา กลุ่มตัวอย่างเป็นผู้ป่วยหลังผ่าตัดเปลี่ยนลิ้นหัวใจเทียมที่รับประทานยาต้านการแข็งตัวของเลือด จำนวน 150 ราย เลือกกลุ่มตัวอย่างด้วยวิธีการสุ่มแบบเฉพาะเจาะจง เก็บรวบรวมข้อมูลระหว่างเดือนมกราคมถึงมีนาคม 2556 โดยใช้แบบสอบถาม ประกอบด้วย แบบสอบถามข้อมูลทั่วไป แบบสอบถามความรู้ในการรับประทานยาต้านการแข็งตัวของเลือด แบบสอบถามการรับรู้ตามแบบแผนความเชื่อด้านสุขภาพ และแบบสอบถามวินัยในการรับประทานยาต้านการแข็งตัวของเลือด วิเคราะห์ข้อมูลโดยสถิติเชิงบรรยายและสถิติทดสอบ Mann-Whitney U Test

ผลการวิจัยพบว่า กลุ่มตัวอย่างมีวินัยในการรับประทานยาต้านการแข็งตัวของเลือดอยู่ในระดับสูง ร้อยละ 52.7 และระดับปานกลาง ร้อยละ 47.3 โดยในกลุ่มที่มีวินัยในการรับประทานยาระดับปานกลางพบว่า ปัจจัยการรับรู้อุปสรรคมีความสัมพันธ์ทางลบในระดับปานกลางกับวินัยในการรับประทานยาต้านการแข็งตัวของเลือด อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ส่วนปัจจัยคัดสรรอื่นๆไม่มีความความสัมพันธ์กับวินัยในการรับประทานยาต้านการแข็งตัวของเลือดในกลุ่มตัวอย่าง ส่วนกลุ่มที่มีวินัยในการรับประทานยาสูงไม่พบความสัมพันธ์ระหว่างปัจจัยคัดสรรที่ศึกษาและวินัยในการรับประทานยา

ผลจากการศึกษานี้ พยาบาลควรให้ความสำคัญกับการรับรู้อุปสรรคในการรับประทานยาต้านการแข็งตัวของเลือด และควรให้การช่วยเหลือสนับสนุนด้านต่างๆเพื่อลดการรับรู้อุปสรรค และเพื่อส่งเสริมวินัยในการรับประทานยาต้านการแข็งตัวของเลือด
