Abstract

Elderly with chronic disease may lead to serious complications and death. Impacts on physical, mental, social, and economy may lead to lower quality of life. The sufficiency health promotion is suited to the context of society, culture, belief, and lifestyle. It would help the elderly promote quality of life. This research is a cross – sectional clinical trial study aims to analyze the cost - effectiveness of the implementation of sufficiency health promotion program among elders with chronic diseases in Sub-urban context, Yasothon Province. The samples were 170 elderly with chronic diseases divided into 2 groups on a voluntary basis. The experiment group of 85 people attended the sufficiency health promotion program and control group of 85 people got standard health promotion services according to elderly clinic. The instruments used in data collection consists of cost data sheet from health care provider and clients view. A quality of life data were collected before and after participating in the program measured by WHOQOL-BREF-THAI. Cost effectiveness evaluation from the costs invested to increase one level of quality of life in each person was estimated.

                It was found that the elder persons who participated in experiment group had increase more quality of life than that of the control group significantly. Costs of sufficiency health promotion program in health care provider view and clients view were 66,500 baht and 35,000 baht, respectively. While the costs of standard health promotion services according to elderly clinic in health care provider view and clients view were 42,750 baht and 14,300 baht, respectively. 17 persons in experiment group and 2 persons in control group could increase one level of quality of life effectively. Cost - effectiveness in experiment group and control group in health care provider view were 3,911.76 baht and 3,911.76 baht and in clients view were 2,058.82 baht and 7,150 baht, respectively.
                  Conclusion:  The cost-effectiveness in both provider and patient perspectives of elderly who participated in the sufficiency health promotion program are more than the elderly who received regular care.
