การพัฒนาระบบการดูแลผู้ป่วยโรคมะเร็งระยะสุดท้ายแบบประคับประคอง ในโรงพยาบาลสรรพสิทธิประสงค์ อุบลราชธานี

บทคัดย่อ

This research is the researching and development for developing the palliative care for patient with end stage of cancer system in Sunpasitthiprasong, Ubon Ratchathani by studying at the ward of  Prapathumworaratsuriyawong 5^th, Chemotherapy ward, medicine ward, Gynecology ward, Chest and Vasculatory ward and the ward of children 5^th. The main informants are caregivers, and patients who are at the end stage of cancer which all total is 60 people. For the informants who are the health team includes physicians, cancer specialists, nurses, pharmacists, psychological nurses, nutritionists, physiotherapists, almoners, and nurses from social medicine department. The information gathered by noticing, interviewing, and having group conversation. The period of studying was from October 2010 to December 2012. The equipment used are caregiver interview form, personnel interview form and questionnaires that established by our  team , analyzing the qualitative data from the content, and analyzing the quantitative data from the frequency,percentage, average and standard deviation.  For the research result, the system of palliative care for patient with end stage of cancer in Sunpasitthiprasong Hospital is linked together since the patients were diagnosed the disease until after the patients died. Patients and their families are taken care well and thoroughly. The preparation in the finality and making decision to die naturally are considered. There is also the Multidisciplinary team to take care of patients, the care from nurses for case management, and the equipment and care supporting system. The organization has the system of palliative care for patient with end stage of cancer which can response to the problem and the requirement of the user. The system is appropriate and related with the context, society and culture which can help the patients who are at the end stage of cancer and their families in high quality of the professional standard.The result found that 100 percent of the caregivers are satisfied to take care of patients, the personnel’s opinion in the quality of taking care within all departments is in the very good level, and the self-evaluation about the efficiency of palliative care of patients is in the good level..

การวิจัยครั้งนี้เป็นการวิจัยและพัฒนา   เพื่อพัฒนาระบบการดูแลผู้ป่วยโรคมะเร็งระยะสุดท้ายแบบประคับประคองในโรงพยาบาลสรรพสิทธิประสงค์  อุบลราชธานี  ศึกษาในหอผู้ป่วยพระปทุมวรราช         สุริยวงศ์  5  หอผู้ป่วยเคมีบำบัด  หอผู้ป่วยอายุรกรรม  3  หอผู้ป่วยนรีเวชกรรม  หอผู้ป่วยทรวงอกและหลอดเลือด  และหอผู้ป่วยเด็ก 5 ของโรงพยาบาลสรรพสิทธิประสงค์   ผู้ให้ข้อมูลหลักประกอบด้วย  ผู้ป่วยผู้ดูแลและผู้ป่วยโรคมะเร็งระยะสุดท้าย จำนวน 60 ราย  บุคลากรทีมสุขภาพ ประกอบด้วย  อายุรแพทย์ผู้เชี่ยวชาญโรคมะเร็ง พยาบาล  เภสัชกร พยาบาลจิตวิทยา โภชนากร นักกายภาพบำบัด นักสังคมสงเคราะห์ พยาบาลจากกลุ่มงานเวชกรรมสังคม รวบรวมข้อมูลโดย การสังเกต  สัมภาษณ์  และสนทนากลุ่ม  ระยะเวลาในการศึกษา  ตุลาคม 2553 ถึง ธันวาคม 2555  เครื่องมือที่ใช้  ได้แก่  แบบสัมภาษณ์ผู้ดูแลและบุคลากร  รวมทั้งแบบประเมินที่พัฒนาขึ้น การวิเคราะห์ข้อมูลเชิงคุณภาพ  ใช้การวิเคราะห์เนื้อหา  ข้อมูลเชิงปริมาณวิเคราะห์โดยใช้ ความถี่ ร้อยละ  ค่าเฉลี่ย  ส่วนเบี่ยงเบนมาตรฐาน  ผลการวิจัย  ระบบการดูแลผู้ป่วยโรคมะเร็งระยะสุดท้ายแบบประคับประคองในโรงพยาบาลสรรพสิทธิประสงค์  อุบลราชธานี  มีความต่อเนื่องตั้งแต่ผู้ป่วยถูกวินิจฉัยโรคมะเร็งระยะสุดท้ายจนถึงหลังการสูญเสีย  ผู้ป่วยและครอบครัวได้รับการดูแลที่ครอบคลุมองค์รวม   ได้มีการเตรียมตัวในวาระสุดท้ายและมีการตัดสินใจที่จะเสียชีวิตตามกระบวนการธรรมชาติมากขึ้น มีทีมสหวิชาชีพในการดูแลผู้ป่วย  มีรูปแบบการดูแลทางการพยาบาลแบบรายกรณี  มีเครื่องมือและระบบสนับสนุนการดูแล องค์กรได้ระบบการดูแลผู้ป่วยโรคมะเร็งระยะสุดท้ายแบบประคับประคองที่ตอบสนองปัญหา  ความต้องการของผู้ใช้บริการ  มีความเหมาะสม  สอดคล้องกับบริบท สังคมและวัฒนธรรม  ที่ช่วยให้ผู้ป่วยโรคมะเร็งระยะสุดท้ายและครอบครัว ได้รับการดูแลอย่างมีคุณภาพตามมาตรฐานวิชาชีพ  และมีความพึงพอใจ เกิดผลลัพธ์  คือ  ผู้ดูแลมีความพึงพอใจในการดูแลโดยรวมในระดับมากที่สุดร้อยละ 100  ความคิดเห็นของบุคลากรทางการพยาบาลต่อคุณภาพการดูแลภายในหน่วยงานในทุกด้านอยู่ในระดับดีมาก  การประเมินตนเองเกี่ยวกับประสิทธิภาพการดูแลผู้ป่วยแบบประคับประคองอยู่ในระดับดี
