การวิจัยเชิงปฏิบัติการนี้ มีวัตถุประสงค์เพื่อพัฒนารูปแบบการพยาบาลหญิงตั้งครรภ์เพื่อป้องกันภาวะตกเลือดหลังคลอดระยะแรก และศึกษาผลลัพธ์ของการนำรูปแบบที่พัฒนาแล้วมาใช้ในการพยาบาลหญิงตั้งครรภ์ ในโรงพยาบาลสรรพสิทธิประสงค์ อุบลราชธานี ผู้ร่วมวิจัยประกอบด้วย หญิงตั้งครรภ์ที่มาคลอด 220 คน พยาบาลวิชาชีพที่ปฏิบัติงานในงานฝากครรภ์ งานห้องคลอด รวม 16 คน รวบรวมข้อมูลโดยการสังเกต สัมภาษณ์ สนทนากลุ่ม และใช้แบบสอบถาม ระหว่างเดือนตุลาคม 2558 ถึง พฤษภาคม 2560 ดำเนินการวิจัยเป็นการวิจัยเชิงปฏิบัติการของ Kemmis & McTaggart มี 4 ขั้นตอน ได้แก่ 1) วิเคราะห์สถานการณ์ ปัญหา 2) ลงมือปฏิบัติตามแผน 3) สังเกตการปฏิบัติตามแผน 4) สะท้อนผลการปฏิบัติ ติดตามประเมินผล  นำข้อมูลมาพิจารณาร่วมกัน วิเคราะห์ข้อมูลเชิงปริมาณ โดยใช้ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และข้อมูลเชิงคุณภาพใช้การวิเคราะห์เชิงเนื้อหา

ผลการวิจัย พบว่า รูปแบบการพยาบาลหญิงตั้งครรภ์เพื่อป้องกันภาวะตกเลือดหลังคลอดระยะแรก ประกอบด้วย 1) ทีมพยาบาลผู้จัดการรายกรณี 2) เครื่องมือที่ใช้ในการดูแลหญิงตั้งครรภ์ ได้แก่ การประเมินความเสี่ยงต่อภาวะตกเลือดหลังคลอดระยะแรก  แนวปฏิบัติการพยาบาลหญิงตั้งครรภ์เพื่อป้องกันภาวะตกเลือดหลังคลอดระยะแรก และถุงตวงเลือด  3) ระบบการติดตามดูแลต่อเนื่อง 24 ชั่วโมง และติดตามเยี่ยมบ้านกรณีที่มีภาวะแทรกซ้อนรุนแรง ผลของการพัฒนารูปแบบ ทำให้พยาบาลปฏิบัติตามแนวปฏิบัติของพยาบาล อยู่ในระดับ ดี  โดยเฉพาะการประเมินความเสี่ยงต่อภาวะตกเลือดหลังคลอด และการนวดคลึงมดลูก คะแนนความรู้ของพยาบาล เพิ่มขึ้นจากเดิม ร้อยละ 66.67 เป็น ร้อยละ 91.13  ระดับความคิดเห็นของพยาบาลต่อการใช้รูปแบบที่พัฒนาขึ้นโดยรวมอยู่ในระดับมาก  หญิงตั้งครรภ์ พบว่า มีภาวะตกเลือดหลังคลอดระยะแรก ร้อยละ 22.7 ไม่มีอุบัติการณ์ตัดมดลูก และเสียชีวิตจากภาวะตกเลือดหลังคลอด

ดังนั้นการพัฒนารูปแบบการพยาบาลหญิงตั้งครรภ์เพื่อป้องกันการตกเลือดหลังคลอด จึงเป็นแนวทางสำหรับพยาบาลในการดูแลหญิงตั้งครรภ์ไม่ให้เกิดภาวะตกเลือดหลังคลอด และภาวะแทรกซ้อนที่รุนแรงได้

Abstract

This Action research aimed to develop a nursing model for Pregnant Women to Prevention of Early Postpartum  Hemorrhage (PPH) and to study outcome of the developed model in Sunpasitthiprasong Hospital, UbonRatchathani. Study participants included 220 pregnants and 16 register nurses including prenatal care unit and Labour room. Data were collected by observations, in-depth interviews, focus - group discussion and questionnaire during 1 October 2015 to 31 May 2017. Research regulation of Kemmis & McTaggart was used in operating procedure, included 4 steps; 1) Analyzing problem situation and gathering 2) Creating the plan 3) Noticing and performing 4) Reflecting the result of the operation: This step was preceded by collecting and analyzing data of the performance as the CNPG. The data were analyzed by using content analysis for qualitative data and descriptive statistics for quantitative data.

The study revealed that the nursing model for preventing early PPH consisted of 1) case manager nurse team 2) pregnant care tool such as: screening of risk factor associated to PPH in pregnant, the CNPG for preventing early PPH and the collector bag for the measurement postpartum blood loss 3) immediate PPH care. The results found that the practice for early PPH as the CNPG was in a good level ( X= 2.0). The knowledge of nurses was in a high level , as well as opinion of register nurse to the model  was at good level (x =3.89) There was no incidence of maternal death, and hysterectomy from the PPH. After using the model, the satisfaction of pregnant was high level (X = 3.93) .

In conclusion, the nursing model for Pregnant Women to Prevention of Early PPH should be implemented to prevent the early PPH, especially for a serious complication of PPH.
