The development of instructional media in the modern era must prioritize enabling learners to independently study, understanding, and internalize knowledge to create transformative learning. This research and development aimed to develop multimedia through online instruction using transformative learning process in the primary medical care course, compare students' knowledge before and after using multimedia, and assess students' satisfaction. The sample consisted of 116 fourth-year nursing students. The research instruments comprised a multimedia-based learning model, a satisfaction questionnaire, and a knowledge test on primary medical care, with reliability coefficients of .81 (Cronbach's alpha) and .90 (Kuder-Richardson 20), respectively. Data were analyzed using descriptive statistics and paired sample t-tests. The results found that the multimedia through online instruction had overall satisfaction levels in the appropriateness of multimedia (μ=4.55, σ=0.58) and multimedia efficiency (μ=4.48, σ=0.62) at the highest level. Students' knowledge after using the multimedia through online instruction with transformative learning process in the primary medical care course was significantly higher than before learning (p<.01). The developed multimedia through online instruction using transformative learning process in the primary medical care course could be implemented in teaching and learning to help students develop their knowledge independently and continuously.
