การรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยของบุคลากรในหน่วยไตเทียม ในเขตกรุงเทพมหานคร

บทคัดย่อ
ความปลอดภัยของผู้ป่วยเป็นปัญหาสำคัญและส่งผลกระทบต่อระบบบริการสุขภาพ การวิจัยเชิงสำรวจแบบภาคตัดขวาง มีวัตถุประสงค์เพื่อศึกษาระดับการรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยของบุคลากรในหน่วยไตเทียมในเขตกรุงเทพมหานคร กลุ่มตัวอย่างคือบุคลากรที่ปฏิบัติงานในหน่วยไตเทียมจำนวน 366 คน เก็บรวบรวมข้อมูลตั้งแต่เดือนมีนาคม - กรกฎาคม 2567 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา การวิเคราะห์ความแปรปรวน ไคสแควร์ และการวิเคราะห์ถดถอยโลจิสติกแบบไบนารี
ผลการวิจัยพบว่า ระดับการรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยของบุคลากรในหน่วยไตเทียมโดยรวมมีระดับการรับรู้สูง มีค่าคะแนนเฉลี่ย 4.18 คะแนน เมื่อจำแนกตามประเภทของหน่วยไตเทียม ทั้ง 4 ประเภท คือ 1) ดำเนินการโดยสถานพยาบาลภาครัฐ 2) ดำเนินการโดยหน่วยงานภายนอก 3) ดำเนินการภายนอกโรงพยาบาลหรือคลินิกเวชกรรมเฉพาะทางไตเทียม และ 4) ดำเนินการโดยสถานพยาบาลภาคเอกชน มีระดับคะแนนการรับรู้สูง (ค่าคะแนนเฉลี่ย 4.23, 4.18, 4.17 และ 4.11 ตามลำดับ) เมื่อเปรียบเทียบความแตกต่างของการรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยทั้งสี่ประเภท มีการรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยในภาพรวมไม่แตกต่างกัน (F (3,360) = 1.954, p-value = 1.21) ผลการวิเคราะห์หาความสัมพันธ์ของตัวแปร พบว่า หน่วยไตเทียมที่มีการจัดหาแหล่งข้อมูลเพื่อสนับสนุนการปฏิบัติการด้านความปลอดภัยของผู้ป่วย มีโอกาสรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยในภาพรวมสูงเป็น 5.7 เท่า (95% CI =1.79 - 18.28) เปรียบเทียบกับหน่วยไตเทียมที่ไม่มีการจัดหาแหล่งข้อมูล
แม้จะพบว่ามิติระดับการรับรู้วัฒนธรรมความปลอดภัยผู้ป่วยในภาพรวมอยู่ในระดับสูง ทั้ง 10 ด้าน แต่มี 2 ด้าน ได้แก่ มิติด้านการจัดคนทำงานและความเร่งรีบในการทำงาน และมิติด้านการตอบสนองต่อความผิดพลั้งที่มีค่าคะแนนเฉลี่ยต่ำ ดังนั้นผู้บริหารหน่วยไตเทียมควรส่งเสริมนโยบายด้านความปลอดภัยเชิงรุกกระตุ้นให้บุคลากรตระหนักและปฏิบัติอย่างต่อเนื่อง จัดหาแหล่งข้อมูลเพื่อสนับสนุนการปฏิบัติการด้านความปลอดภัยของผู้ป่วย เพื่อส่งเสริมและธำรงมาตรฐานด้านวัฒนธรรมความปลอดภัยผู้ป่วยของหน่วยไตเทียม
