บทคัดย่อ
การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อพัฒนารูปแบบการจัดการดูแลผู้ป่วยโรคติดต่ออันตราย โรคอุบัติใหม่ อุบัติซ้ำ กลุ่มตัวอย่างเป็นพยาบาล และผู้ช่วยเหลือผู้ป่วย เลือกแบบเฉพาะเจาะจง ดำเนินงานวิจัย 4 ระยะ คือ 1) สำรวจสภาพปัญหาและความต้องการ สอบถามจากพยาบาล 140 คน และสัมภาษณ์ผู้บริหาร 15 คน 2) สร้างต้นแบบการจัดการดูแล จากข้อมูลขั้นที่ 1 และประเมินความเหมาะสมของรูปแบบโดยใช้แบบวัด AGREE และทดลองใช้ต้นแบบในสถาบันบำราศนราดูร กลุ่มตัวอย่างเป็นพยาบาล 15 คนผู้ช่วยเหลือฯ จำนวน 6 คน 3) ปรับปรุงต้นแบบให้สมบูรณ์ และนำไปใช้ที่โรงพยาบาล 2 แห่ง กับพยาบาล 15 คน และผู้ช่วยเหลือ 4 คน และ 4) ประเมินประสิทธิผลของต้นแบบ โดยนำต้นแบบไปใช้ในสถาบันบำราศฯ กับพยาบาล 30 คน ผู้ช่วยเหลือฯ 10 คน วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้ ค่าเฉลี่ย และใช้การสรุปประเด็นจากผลการสัมภาษณ์และคำถามปลายเปิด
ผลการศึกษาพบปัญหา 1) อัตรากำลัง สถานที่และสิ่งแวดล้อม และเครื่องมืออุปกรณ์ในการดูแลผู้ป่วยไม่เพียงพอ 2) รูปแบบที่สร้างขึ้น เรียก EID Model ผลการประเมินรูปแบบมีความเหมาะสม มากกว่าร้อยละ 70 ในทุกด้าน เน้นกิจกรรมที่ต้องปฏิบัติ การจัดสภาพสิ่งแวดล้อม การใส่อุปกรณ์ป้องกันร่างกายตามมาตรฐานการแยกผู้ป่วย รวมทั้งการพัฒนาส่วนต่างๆที่เกี่ยวข้อง ผลการนำ EID Model ไปทดลองใช้พบความพึงพอใจอยู่ในระดับมาก มีข้อเสนอแนะให้เพิ่มเรื่องการดูแลเน้นการดูแลด้านจิตใจ นำผลมาปรับรูปแบบ 3) รูปแบบใหม่คือ 2EID Care Model มี 5 องค์ประกอบ (1) การจัดการระบบการดูแล (2) กิจกรรมและแผนที่ต้องเตรียมปฏิบัติ (3) การจัดการสถานที่และสิ่งแวดล้อม (4) การปฏิบัติเพื่อป้องกันการแพร่กระจายเชื้อ และ (5) การพัฒนาส่วนต่างๆ ที่เกี่ยวข้อง ผลความพึงพอใจอยู่ในระดับมาก มีข้อเสนอแนะเรื่องอัตรากำลังและสมรรถนะการดูแลผู้ป่วยวิกฤต และ 4) ใช้ 2EID Care Model พบความพึงพอใจอยู่ในระดับมาก มีข้อเสนอแนะเรื่องการดูแลผู้ป่วยระยะสุดท้ายปฏิบัติได้น้อย
