เปรียบเทียบประสิทธิภาพการเจาะเนื้อเยื่อในรอยโรคใต้เยื่อบุผิวและเนื้องอกตับอ่อนระหว่างเข็มเจาะตรวจ เบอร์ 19 กับเข็มเจาะตรวจเบอร์ 22 ผ่านการส่องกล้องที่ติดอัลตราซาวน์

บทคัดย่อ

ภูมิหลัง: การส่องกล้องอัลตราซาวนด์ร่วมกับการเจาะเก็บตัวอย่างเนื้อเยื่อเป็นวิธีสำคัญในการวินิจฉัยรอยโรคใต้เยื่อบุผิวและเนื้องอกตับอ่อน โดยขนาดของเข็มอาจมีผลต่อความเพียงพอ  ของเนื้อเยื่อและประสิทธิภาพในการวินิจฉัย วัตถุประสงค์: เพื่อเปรียบเทียบประสิทธิภาพการเก็บเนื้อเยื่อระหว่างเข็มขนาด 19-gauge และ 22-gauge วิธีการ: เป็นการศึกษาเชิงสังเกตแบบเปรียบเทียบในผู้ป่วยที่เข้ารับการส่องกล้องอัลตราซาวนด์ร่วมกับการเจาะเก็บตัวอย่างเนื้อเยื่อที่โรงพยาบาลมะเร็งลพบุรี ระหว่างวันที่ 1 ตุลาคม 2567 ถึง 30 ตุลาคม 2568 จำนวน 72 ราย แบ่งเป็นกลุ่มเข็ม 19-gauge จำนวน 15 ราย และ 22-gauge จำนวน 57 ราย เก็บข้อมูลลักษณะรอยโรค จำนวนครั้งที่เจาะ ระยะเวลาทำหัตถการ ผลทางพยาธิวิทยา และความเพียงพอของเนื้อเยื่อ วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและเปรียบเทียบความแตกต่างระหว่างกลุ่ม ด้วย Fisher’s exact test ผล: พบว่ากลุ่มเข็ม 19-gauge และ 22-gauge มีความเพียงพอของเนื้อเยื่อต่อการวินิจฉัยร้อยละ 93.3 และ 80.7 ตามลำดับ โดยไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (OR 3.35, 95% CI 0.40–28.25, P=0.438) ระยะเวลาเฉลี่ยในการทำหัตถการของกลุ่ม 19-gauge เท่ากับ 31.53±12.45 นาที และกลุ่ม 22-gauge เท่ากับ 34.67±11.76 นาที ซึ่งไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P=0.291) การวิเคราะห์เพิ่มเติมโดยใช้จุดตัดที่ 40 นาที พบว่าสัดส่วนของหัตถการที่ใช้เวลาน้อยกว่า 40 นาที แตกต่างกันระหว่างสองกลุ่ม (P=0.028) ซึ่งจุดตัดดังกล่าวไม่ได้กำหนดไว้ล่วงหน้าในแผนการวิเคราะห์ สรุป: การเจาะตรวจเนื้อเยื่อด้วยเข็มขนาด 19-gauge และ 22-gauge มีประสิทธิภาพในการเก็บตัวอย่างเนื้อเยื่อเพื่อการวินิจฉัยไม่แตกต่างกันอย่างมีนัยสำคัญ แม้เข็ม 19-gauge มีแนวโน้มให้ตัวอย่างเนื้อเยื่อเพียงพอมากกว่าและมีระยะเวลาทำหัตถการเฉลี่ยสั้นกว่าเล็กน้อย แต่ยังไม่สามารถสรุปได้ว่าเหนือกว่าเข็ม 22-gauge การเลือกใช้ขนาดเข็มจึงควรพิจารณาร่วมกับลักษณะและตำแหน่งของรอยโรค ความเหมาะสมของผู้ป่วย และประสบการณ์ของผู้ปฏิบัติ
