การศึกษาจากโลกแห่งความเป็นจริงแบบย้อนหลังเชิงสังเกต เพื่อประเมินความปลอดภัยและประสิทธิผลของการใช้ยา Fulvestrant (Eranfu) แบบเดี่ยวในผู้ป่วยมะเร็งเต้านมระยะลุกลามชนิด HR+/HER2-

บทคัดย่อ
บทคัดย่อภูมิหลัง: ฟูลเวสแทรนต์ (Fulvestrant) ซึ่งเป็นยาสลายตัวตัวรับฮอร์โมนเอสโตรเจนชนิดเลือกจำเพาะ (Selective Estrogen Receptor Degrader: SERD) เป็นการรักษาด้วยฮอร์โมนที่แนะนำสำหรับผู้ป่วยมะเร็งเต้านมระยะลุกลามชนิด HR-positive, HER2-negative (ABC) แม้ว่าการใช้ร่วมกับยากลุ่มเป้าหมายจะได้รับการอนุมัติแล้ว แต่การใช้ฟูลเวสแทรนต์แบบเดี่ยวยังคงเป็นแนวทางที่ใช้กันอย่างแพร่หลายในเวชปฏิบัติจริง อย่างไรก็ตามหลักฐานจากการใช้จริงในบริบทที่มีภาระโรคสูง เช่น ประเทศไทย ยังมีจำกัด วัตถุประสงค์: เพื่อประเมินความปลอดภัยและประสิทธิผลทางคลินิกของฟูลเวสแทรนต์แบบสามัญ (Eranfu) ในการรักษาแบบเดี่ยวในผู้ป่วยไทยที่เป็นมะเร็งเต้านมชนิด HR+/HER2- ระยะลุกลามในสายการรักษาต่าง ๆ วิธีการ: การศึกษาย้อนหลังจากเวชระเบียนของสตรีวัยหมดประจำเดือนอายุ ≥18 ปี จำนวน 74 ราย ที่ได้รับการรักษาด้วยฟูลเวสแทรนต์ระหว่างวันที่ 1 มกราคม 2565 ถึง 31 มกราคม 2567 โดยมีข้อมูลของผู้ป่วยที่เข้าเกณฑ์จำนวน 29 ราย (ไม่เคยได้รับ ET มาก่อน หรือมีการกลับเป็นซ้ำหลังการรักษาเสริมด้วยยาต้านฮอร์โมน หรือมีการลุกลามของโรคขณะได้รับยาต้านฮอร์โมน และมีข้อมูลติดตามอย่างน้อย 6 เดือน) ข้อมูลที่รวบรวมได้รวมถึงข้อมูลประชากรผู้ป่วย วันที่ลุกลามของโรคและเสียชีวิต การตอบสนองต่อการรักษาหลังเริ่มฟูลเวสแทรนต์แบบเดี่ยว (4 รายในสายแรก, 15 รายในสายที่สอง, และ 10 รายในสายที่สามขึ้นไป) จุดสิ้นสุดหลักคือความปลอดภัย โดยใช้เกณฑ์ CTCAE v5.0 จุดสิ้นสุดรอง ได้แก่ ระยะเวลาปราศจากการลุกลามของโรค (PFS), การรอดชีวิตโดยรวม (OS), และอัตราการตอบสนองโดยรวม (ORR) ผลการศึกษา: อายุมัธยฐานของผู้ป่วยคือ 61 ปี (ช่วง 43–89 ปี) โดย 86.2% (n = 25) มีสถานะการทำงานตามเกณฑ์ Eastern Cooperative Oncology Group ที่ระดับ 0–1 กระดูกเป็นตำแหน่งที่มีการแพร่กระจายมากที่สุด (55.2%, n = 16) และ 15 ราย (51.7%) มีการแพร่กระจายไปยังอวัยวะภายใน ผู้ป่วยได้รับการรักษาด้วยฮอร์โมนหรือเคมีบำบัดมาแล้วสูงสุด 2 สาย การวิเคราะห์ความปลอดภัยโดยรวมพบว่าฟูลเวสแทรนต์มีความทนต่อยาได้ดี โดยมีเหตุการณ์ไม่พึงประสงค์ระดับ ≥4 เพียง 8 ราย PFS มัธยฐานของประชากรทั้งหมดคือ 3.67 เดือน โดยสายแรกมีระยะเวลานานที่สุด (6.73 เดือน) เทียบกับสายที่สอง (3.27 เดือน) และสายหลังจากนั้น (4.27 เดือน) OS มัธยฐานคือ 30.77 เดือน ORR โดยรวมคือ 14.2% และในสายแรกคือ 50% สรุป: ฟูลเวสแทรนต์แสดงให้เห็นถึงผลลัพธ์ที่มีประสิทธิภาพและสามารถทนต่อยาได้ดีในผู้ป่วยไทยที่เป็นมะเร็งเต้านมชนิด HR+/HER2- ระยะลุกลาม รวมถึงผู้ที่มีการแพร่กระจายไปยังอวัยวะภายในและเคยได้รับการรักษาด้วยฮอร์โมนมาก่อน ผลลัพธ์ที่ดีขึ้นในสายแรกเน้นย้ำถึงความสำคัญของการเริ่มรักษาแต่เนิ่น ๆ สนับสนุนบทบาทของฟูลเวสแทรนต์ในสายการรักษาต่าง ๆ โดยเฉพาะในบริบทที่มีข้อจำกัดด้านทรัพยากร
