บทคัดย่อ
Pseudomyxoma Peritonei (PMP) เป็นกลุ่มอาการทางคลินิกที่พบได้ไม่บ่อย มีลักษณะเด่นคือ การสะสมของมูกในปริมาณมากภายในช่องท้อง ซึ่งส่วนใหญ่มีต้นกำเนิดจากเนื้องอกชนิดมูกของไส้ติ่ง (appendiceal mucinous neoplasm) ที่มีการแตกออก การดำเนินโรคที่เป็นไปอย่างช้า ๆ และอาการที่ไม่จำเพาะเจาะจงในระยะแรก ทำให้การวินิจฉัยมีความท้าทาย และผู้ป่วยมักมาพบแพทย์เมื่อโรคมีการลุกลามไปมากแล้ว บทความนี้มีวัตถุประสงค์เพื่อทบทวนวรรณกรรมที่ครอบคลุมถึงความสำคัญพยาธิกำเนิด ลักษณะทางคลินิก การจำแนกประเภททางพยาธิวิทยา ลักษณะทางโมเลกุล แนวทางการรักษาหลักในปัจจุบัน ซึ่งมุ่งเน้นการรักษาแบบผสมผสาน โดยมีศัลยกรรมเพื่อลดปริมาณเนื้องอกให้ได้มากที่สุดร่วมกับการให้ยาเคมีบำบัดอุณหภูมิสูงภายในช่องท้อง (cytoreductive surgery and hyperthermic intraperitoneal chemotherapy) เป็นหลักซึ่งแสดงให้เห็นถึงผลลัพธ์ระยะยาวที่ดีในผู้ป่วยที่ได้รับการคัดเลือกอย่างเหมาะสม และกล่าวถึงแนวทางการรักษาอื่น ๆ โดยเน้นความจำเป็นของการวิจัยเพิ่มเติม เพื่อประเมินบทบาทของวิธีการรักษาใหม่เหล่านี้ และเพื่อพัฒนาแนวทางการรักษาเฉพาะบุคคลให้มีประสิทธิภาพมากขึ้นสำหรับโรคที่พบได้ยากนี้
