A Failure Mode and Effects Analysis for Risk Management in Emergency Room of a District Hospital in Chanthaburi

Abstract

     Currently, the number of accident and emergency service users has increased. Resulting in staff unable to take care of patients thoroughly and waiting the doctor period is longer. Directly affecting the safety of patients. To make the hospital aware of the importance of preventing risks that may occur to the work process and the availability of patient services.

     The purposes of this research are to 1) To analyze failure mode and effects that may occur in the operational process of the emergency room 2) To evaluate and prioritize the problems that may occur in the operation process of the emergency room 3) to guidelines for creating a risk management plan that may occur in the operation of the emergency room according to priority. Application of the FMEA technique in analysis and identify the problems including assessments to prioritize problems of the medical records section, the patient screening process and the emergency room.

     The results revealed all three processes found 11 problems. Three Highest-Ranked of problem that found in the emergency room are 1) Emergency car accident has 81 value of RPN 2) Providing patient services in the event of an emergency, not in a timely has 70 value of RPN and 3) Non-emergency patients come to receive a lot of services outside office hours has 60 value of RPN. Which the member team jointly propose solutions to the problem classified by the cause all of the 11 problems.
