รายงานผู้ป่วย : Blunt Diaphragmatic Injury ที่โรงพยาบาลพระนครศรีอยุธยา

บทคัดย่อ

     Blunt diaphragmatic injury เป็นการบาดเจ็บที่พบได้น้อย และมักวินิจฉัยโรคไม่ได้ รายงานนี้นำเสนอผู้ป่วยชายไทย อายุ 22 ปี ประสบอุบัติเหตุโดนเหล็กหนัก 20 ตันทับบริเวณหน้าอก, ท้องและอุ้งเชิงกราน วินิจฉัยว่ามี intraabdominal injury ทำผ่าตัด explore laparotomy พบเลือดออกในช่องท้อง ไม่มี active bleeding หลังผ่าตัด 3 วัน chest X-ray พบ abnormal air ที่ left lower chest ผลตรวจ CT chest และ abdomen สงสัยว่ามี diaphragmatic injury จึงผ่าตัด reexplore laparotomy พบ left diaphragmatic tear ที่บริเวณ crus และ total gastric herniation ได้ทำ stomach reduction และ repair diaphragm ผู้ป่วยมีการบาดเจ็บร่วมที่สำคัญคือ pseudoaneurysm ของ right hepatic artery และ open book pelvic
fracture ได้ทำ angiogram และ embolization หลังจากผ่าตัดผู้ป่วยมาติดตามการรักษามาเป็นเวลา 2 ปี 8 เดือน ไม่พบว่ามีภาวะแทรกซ้อน
