The Effectiveness of Group-Based Multicomponent Cognitive Training compared with Individual Cognitive Training in Older Adults with Mild Neurocognitive Disorder

Abstract

Objective: To evaluate the effectiveness of group-based multicomponent cognitive training compared with individual cognitive training in older adults with mild neurocognitive disorder at the Geriatric Clinic of Saraburi Hospital. Methods: This randomized controlled trial enrolled 154 older adults diagnosed with mild neurocognitive disorder. Participants were randomly assigned to either an intervention group receiving group-based cognitive training by a multidisciplinary team (n=76) or a control group receiving individual cognitive training by an occupational therapist (n=78). Cognitive performance was evaluated over a 12-month follow-up period using the Thai Mental State Examination (TMSE) and the Montreal Cognitive Assessment (MoCA). Results: Baseline characteristics were comparable between the intervention and control groups, including mean age (75.36 ± 6.44 vs. 74.21 ± 6.63 years), female sex (73.68% vs. 66.67%), marital status (42.11% vs. 51.28%), and no current occupation (89.47% vs. 85.90%). The most common geriatric syndromes across both groups included oral problems (81.58% vs. 74.36%), visual impairment (56.58% vs. 58.97%), and a history of falls (30.26% vs. 38.46%). At 12 months, the intervention group showed no significant within-group improvement in TMSE (mean difference 0.15; 95% CI −0.72 to 1.01; p = 0.738), with no significant between-group difference (mean difference −0.69; 95% CI −1.96 to 0.56; p = 0.279). MoCA scores increased significantly within the intervention group (mean difference 2.18; 95% CI 1.17 to 3.19; p < 0.001) and showed a trend toward greater improvement than in the control group (mean difference 1.17; 95% CI −0.41 to 2.72; p = 0.147), though the between-group difference was not statistically significant. Conclusion: Group-based cognitive training delivered by a multidisciplinary team produced cognitive outcomes that did not differ significantly from individual cognitive training delivered by an occupational therapist, for both TMSE and MoCA scores at 12 months, although the group-based arm showed a statistically significant within-group improvement in MoCA. Group-based cognitive training represents a viable option that reduces the burden on specialized personnel and can be scaled up to geriatric clinics and primary care units (subdistrict health-promoting hospitals) to improve access to care for older adults with mild neurocognitive disorder.
