Blunt Diaphragmatic Injury at Phra Nakhon Si Ayutthaya hospital : Case Report

Abstract

     Blunt diaphragmatic injury is rare and easily missed. A 22-year-old man was hit by 20-ton iron bar. The diagnosis of intraabdominal injury was established. Exploratory laparotomy revealed intraabdominal blood without active bleeding. Three days after the operation, chest X-ray showed abnormal air at left lower chest. Chest and abdominal computed tomography scan was done, the finding suggested diaphragmatic injury. Hence, reexploratory laparotomy was performed. Diaphragmatic tear at left crus and total gastric herniation was found. Stomach was reduced and diaphragmatic tear was repaired. The patient also had pseudoaneurysm of hepatic artery and open book pelvic fracture requiring angiogram and embolization. Two years and eight months after the operation, no complications were discovered.
