Primary Care Service System in Urban Setting: A case Study in Bangkok Area, B.E. 2560

Abstract

     It is a quality study using in-depth interview, focus groups and Delphi technique. Many groups of experts and health workers are involved. There are 3 aspects of problems found. Those are health service aspect, financial aspect and participation in health service management of parties aspect. Many suggestions are proposed. The service should be operated by area base with 30,000 populations in one primary care cluster. Qualified drug store should be included in the service system. Management information System should be setting up both at national and Bangkok area. Referral system is necessary for
better services. Health promotion, prevention, rehabilitation, and service training should be supported by department concerned. Paying to primary care unit should use integrated capitation including curative, preventive, and promotive and rehabilitative services. Costing is necessary for planning and payment system. Leadership training among people and volunteer is the must to ensure success of the service. Using Traditional Thai Medicine and promote health literacy of the people is the key success factors. Using the new approach of quality of life development committee in district level will increase better outcome of the service. The ultimate goal of the primary care system in Bangkok is to serve good quality, effectives and accessibility of the service to the people.
