ภูมิปัญญาของหมอกระดูกพื้นบ้านในการรักษาผู้สูงอายุกระดูกหัก

การวิจัยเชิงคุณภาพครั้งนี้ มีวัตถุประสงค์เพื่อ ศึกษา รวบรวมภูมิปัญญา รวบรวมสมุนไพรและตำรับยาสมุนไพร และสังเคราะห์องค์ความรู้ของหมอกระดูกพื้นบ้านในการดูแลรักษา ส่งเสริมและฟื้นฟูสุขภาพของผู้สูงอายุ กลุ่มผู้ให้ข้อมูลเป็นหมอนวดพื้นบ้านในประเทศ 4 ภาค ทำการคัดเลือกแบบเจาะจง เครื่องมือในการเก็บรวบรวมข้อมูลนั้นคณะผู้วิจัยได้ใช้แบบสัมภาษณ์เชิงลึกที่สร้างขึ้น การสังเกต การจดบันทึก การบันทึกเทป และการบันทึกภาพ วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์เนื้อหาแล้วส่งรายงานการวิเคราะห์นั้นกลับไปให้กลุ่มผู้ให้ข้อมูลตรวจสอบความถูกต้องและเพิ่มเติมเนื้อหาได้ตามความเหมาะสม แล้วนำข้อมูลที่ได้รับกลับมาแก้ไขให้ถูกต้องตามความเป็นจริงและเพิ่มเติมข้อมูลตามที่กลุ่มผู้ให้ข้อมูลต้องการเปิดเผยองค์ความรู้และภูมิปัญญา

ผลการวิจัย พบว่า ความรู้เรื่องการรักษาภาวะกระดูกหักของหมอกระดูกพื้นบ้านที่ได้รับมาจากการสืบทอดของบรรพบุรุษ จากครู (ผู้ล่วงลับ) และจากสัมผัสที่หก จึงไม่มีตำราเรียน ใช้วิธีการสอนแบบตัวต่อตัวรวมทั้งใช้ประสบการณ์ของผู้สอนจากการรักษา กระดูกหักที่พบบ่อย ได้แก่ แขนหัก ขาหัก และหัวไหล่หลุด  มีขั้นตอนการรักษาที่สอดคล้องกันตามลำดับ ดังนี้ การซักประวัติ การตรวจร่างกาย การใช้ผลการตรวจของแพทย์แผนปัจจุบัน และการประเมินผลการรักษา นอกจากนี้ยังควบคุมอาหารที่ควรงดบริโภค และเน้นการดูแลสุขภาพผู้สูงอายุแบบองค์รวม ส่วนการรักษาที่แตกต่างกัน ได้แก่ การใช้สมุนไพรร่วมกับน้ำมันประสานกระดูกที่มีสูตรเฉพาะและมีคาถากำกับ การใช้เฝือกไม้ไผ่หรือประยุกต์ใช้ท่อพีวีซี และด้านทัศนคติและความเชื่อ พบว่า กลุ่มผู้ให้ข้อมูลมีทัศนคติที่ดี และภูมิใจต่อการเป็นหมอกระดูกพื้นบ้าน เนื่องจากได้ช่วยเหลือเพื่อนมนุษย์ให้พ้นทุกข์จากโรคและอาการเจ็บป่วย ยินดีที่จะถ่ายทอดความรู้ให้ศิษย์ด้วยความเต็มใจ ส่วนเรื่องความเชื่อเรื่องการบูชาครู และสิ่งศักดิ์สิทธิ์เหนือธรรมชาติ ประกอบด้วย ค่าขันครู และการไหว้ครูประจำปี สำหรับค่าตอบแทนนั้นห้ามเรียกร้องแล้วแต่ศรัทธา

                ข้อเสนอแนะ ด้านนโยบายต้องการให้หมอกระดูกพื้นบ้านที่มีผลการรักษาตามเกณฑ์มาตรฐานได้รับการขึ้นทะเบียนเป็นผู้มีใบประกอบโรคศิลป์ ด้านการปฏิบัติ ต้องการพัฒนาองค์ความรู้ของหมอกระดูกพื้นบ้านให้เป็นไปตามมาตรฐานของการรักษา ส่วนด้านการนำผลไปใช้ประโยชน์ ต้องการให้ทีมสุขภาพใช้ศาสตร์ทางเลือกเพื่อการดูแล ส่งเสริมและฟื้นฟูสุขภาพสำหรับผู้สูงอายุอย่างเป็นรูปธรรมโดยใช้หลักฐานเชิงประจักษ์

The Wisdom of Indigenous Healer’s Chiropractic the Treatment in Elderly Fracture

Abstract

                The purpose of this qualitative research was to study, gathering various wisdom, herbs and herbal recipes, along with analysis of indigenous healer’s chiropractic body knowledge concerning treatment, health promotion and rehabilitation in the elderly fracture. The participants were indigenous healer’s chiropractic randomly assigned into purposive sampling from 4 regions in Thailand. The data were collected by using in-depth interviews, observation, taking notes, taped recording and photographing. The content was analyzed reported and submitted back to the participants for accuracy and adding content as appropriate. Additional data were added as the participants’ desired to reveal their wisdom and body knowledge.

                The results revealed that the treatment of fracture was the indigenous healer’s chiropractic body of knowledge which was inherited from ancestors, teachers (the deceased), and the sixth sense of practitioners. They have no texts but using one by one method and teacher’s experiences. The common fractures were found at extremities and shoulder dislocation. The corresponding characteristics of the treatments and procedures included history taking, physical examination, result of examination from modern medical physician and treatment evaluation. Besides, the controlling of diet and focus on holistic approach for the elderly were practiced. The difference characteristics of treatment methods were using herbs with special oil that aimed to join the bone together and controlled with directed spell, utilizing bamboo cast or PVC tube.

                The characteristics of attitudes and believes were good because the chiropractors were proud to be able to help people who suffering from diseases and illness. They were willing to teach and pass on their knowledge to the learner.  Beliefs about teacher worship and supernatural divinity consisted of paying money for respect the teacher and annual Wai Kru ceremony. The payment cost for learning could not be requested but can give according to individual faith.

                Suggestions:  The policy on the indigenous healer’s chiropractic who had met the required standard level should be registered and licensed for their practice. Concerning the practice; must develop the body knowledge of indigenous healer’s chiropractic according to the treatment standard. The utilization of the result should be used by alternative health team in caring, supporting and rehabilitation in the elderly fracture by using the evidence based.
