การพัฒนาแบบวัดพฤติกรรมการดูแลอย่างเอื้ออาทรของนักศึกษาพยาบาล

                การวิจัยเชิงพรรณนานี้มีวัตถุประสงค์เพื่อพัฒนาแบบวัดพฤติกรรมการดูแลอย่างเอื้ออาทร
ในนักศึกษาพยาบาล วิทยาลัยเครือข่ายภาคใต้ สังกัดกระทรวงสาธารณสุข  การพัฒนาแบบวัด ประกอบด้วย

1) การกำหนดความหมายของพฤติกรรมการดูแลอย่างเอื้ออาทร ของนักศึกษาพยาบาลโดยการทบทวน วรรณกรรม การสนทนากลุ่มพยาบาลและอาจารย์พยาบาล จำนวน 10 คน และการพรรณนาความหมายของ พฤติกรรมการดูแลอย่างเอื้ออาทรโดยนักศึกษาพยาบาล จำนวน 150 คน ได้องค์ประกอบของพฤติกรรม การดูแลอย่างเอื้ออาทร 5 องค์ประกอบ ได้แก่ การเคารพในบุคคล การสื่อสารอย่างมีประสิทธิภาพ การสนับสนุนประคับประคองจิตใจและจิตวิญญาณ การเอาใจใส่ดูแลประดุจญาติ และ สมรรถนะเชิงวิชาชีพ 2) การสร้างแบบวัด มีข้อคำถาม 48 ข้อ ที่ผู้วิจัยพัฒนาขึ้น และ วิเคราะห์ความตรงเชิงเนื้อหาผู้ทรงคุณวุฒิ 5 ท่าน โดยได้ค่าดัชนีความตรงเชิงเนื้อหาเท่ากับ 0.89 แบบวัดฉบับปรับปรุงครั้งที่ 1 จำนวน 49 ข้อ ถูกนำไป ทดสอบความตรงแบบเผชิญหน้า (Face validity) กับนักศึกษาพยาบาลชั้นปี 4 จำนวน 8 คน และอาจารย์นิเทศ จำนวน 10 คน แบบวัดฉบับปรับปรุงครั้งที่ 2 ได้ถูกนำไปทดลองใช้ในนักศึกษาพยาบาลชั้นปี2 (หลักสูตร เจ้าพนักงานสาธารณสุขชุมชน) วิทยาลัยพยาบาลบรมราชชนนี สงขลา 30 คน เพื่อวิเคราะห์รายข้อ(Item
analysis) และทดสอบความเที่ยงของแบบวัด ได้ค่าสัมประสิทธิ์สหสัมพันธ์แบบครอนบาค (Cronbach’s
Alpha coefficient) 0.95 และ 3) การทดสอบคุณภาพแบบวัด กับนักศึกษาพยาบาลชั้นปี 2-4 วิทยาลัยพยาบาล เครือข่ายภาคใต้ 4 แห่ง รวม 513 คน วิเคราะห์ความตรงเชิงโครงสร้างโดยการวิเคราะห์ปัจจัย (Factor
analysis) พบว่า แบบวัดมี 5 องค์ประกอบ ได้แก่ 1) การสนับสนุนประคับประคองจิตใจและจิตวิญญาณ
2) การสื่อสารอย่างมีประสิทธิภาพ  3) สมรรถนะเชิงวิชาชีพ 4) การเอาใจใส่ดูแลประดุจญาติ และ
5) การเคารพในบุคคล ทั้ง 5 องค์ประกอบสามารถอธิบายความแปรปรวนของพฤติกรรมการดูแล อย่างเอื้ออาทรได้ร้อยละ 41.5องค์ประกอบที่ 1, 2, 3, 4, และ 5 สามารถอธิบายความแปรปรวนของ พฤติกรรมการดูแลอย่างเอื้ออาทร ร้อยละ26.5, 4.7, 4.1, 3.2, และ 2.9 ตามลำดับ ความเที่ยงของแบบวัดทั้งฉบับ เท่ากับ .94  และความเที่ยงขององค์ประกอบที่ 1, 2, 3, 4, และ 5 เท่ากับ .89, .82, .81, .75 และ .73 ตามลำดับ

                แบบวัดพฤติกรรมการดูแลอย่างเอื้ออาทรในนักศึกษาพยาบาลฉบับนี้ มีคุณภาพในระดับที่ยอมรับ ได้และมีความเหมาะสมที่จะนำไปใช้ในการวัดพฤติกรรมการดูแลอย่างเอื้ออาทรในนักศึกษาพยาบาล

Abstract

                The aim of this descriptive study was to develop and test psychometric properties of the Nursing Students’ Caring Behavior Scales (NSCBS).  The meaning of nursing students’ caring behaviors,
determined from the result of literature reviews, focus group interview in 6 registered nurses and 4 nurse faculties, and description of 150 nursing students, could be classified into 5 dimensions: respect, effective communication, emotional and spiritual support, compassionated care, and professional competency.  The content validity of the first version of 48-item questionnaire was validated by 5 experts.  The content validity index was reported at 0.89.  The first revised version of the scale included 49 items was tested for a face validity in 8 forth-year-nursing students and 10 nurse educators from 5 nursing specialists.  The item analysis and internal consistency reliability of the second revised version was evaluated in thirty
second- year-nursing students (Public health technician program), Boromarajonnai College of Nursing Songkhla.  All items were kept in the scale and Cronbach’s Alpha coefficient was 0.95.  Then, the scale was tested for construct validity by using exploratory factor analysis by 513 nursing students, 2- to- 4 year from four nursing colleges of the Southern Network Colleges, Ministry of Public Health.  The result showed that the items were five factors: emotional and spiritual support, effective communication, professional
competency, compassionated care, and respect.  All five factors could explain 41.5 % of the total variance of the nursing students’ caring behaviors (NSCB).  Focusing on each factor, the factor 1, 2, 3, 4, and 5 could explain 26.5, 4.7, 4.1, 3.2, and 2.9 of the total variance of the NSCB, respectively.  The internal consistency reliability of the questionnaire was .94 and the reliability of the factor 1, 2, 3, 4, and 5 was .89, .82, .81, .75 and .73 respectively.

                The Nursing Student Caring Behavior Scale demonstrated acceptable psychometric properties and cultural appropriateness for measuring caring behaviors of nursing students in the Southern Network Colleges.
