รูปแบบการจัดการการดูแลเพื่อป้องกันการพลัดตกหกล้มของผู้ป่วยที่มีความพิการ ในหอผู้ป่วย โรงพยาบาลทหารผ่านศึก

การวิจัยและพัฒนา มีวัตถุประสงค์เพื่อสร้างรูปแบบ และศึกษาผลลัพธ์รูปแบบการจัดการการดูแลเพื่อป้องกันการพลัดตกหกล้มของผู้ป่วยที่มีความพิการในหอผู้ป่วย ดำเนินการ 2 ขั้นตอน โดยขั้นตอนที่ 1 การสร้างรูปแบบการจัดการการดูแลเพื่อป้องกันการพลัดตกหกล้ม ผู้ให้ข้อมูลหลักสำหรับการสนทนากลุ่ม คือ ทีมสหสาขาวิชาชีพ ประกอบด้วย แพทย์ ผู้บริหารทางการพยาบาล พยาบาลวิชาชีพ รวม 9 คน และผู้ป่วย จำนวน 9 คน และขั้นตอนที่ 2 ศึกษาผลลัพธ์ของรูปแบบการจัดการการดูแลเพื่อป้องกันการพลัดตกหกล้ม กลุ่มตัวอย่าง คือ พยาบาลวิชาชีพ จำนวน 12 คน และผู้ป่วยที่มีความพิการในหอผู้ป่วย จำนวน 30 คน เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) แนวประเด็นการสนทนากลุ่ม 2) แบบประเมินปัจจัยเสี่ยงต่อการพลัดตกหกล้ม และ 3) แบบประเมินความพึงพอใจของพยาบาลวิชาชีพได้ค่าความตรงเชิงเนื้อหาอยู่ระหว่าง 0.60 - 1.00 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา การวิเคราะห์เชิงเนื้อหา และสถิติค่าไคสแควร์ ผลการวิจัยพบว่า
1. รูปแบบฯ ที่สร้างได้ ประกอบด้วย 1) การตรวจร่างกายและการประเมินสภาพผู้ป่วยพิการ 2) การระบุกลุ่มผู้ป่วยและการประเมินปัจจัยเสี่ยงต่อการพลัดตกหกล้มในผู้ป่วยแต่ละรายเพื่อวางแผนในการดูแล 3) การปฏิบัติหรือให้การดูแลตามแผนที่วางไว้ 4) การติดตามประเมินผล และ 5) การส่งเสริมรูปแบบฯ ไปในเชิงนโยบาย
2. อัตราการพลัดตกหกล้มหลังการใช้รูปแบบฯ พบว่า หลังใช้รูปแบบฯ ต่ำกว่าก่อนใช้รูปแบบฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .001 และ หลังการใช้รูปแบบฯ พบว่าพยาบาลวิชาชีพมีความพึงพอใจในระดับมากที่สุด (M = 4.88, SD = 0.18)
ดังนั้นควรนำรูปแบบฯ นี้ไปใช้ในโรงพยาบาลที่มีบริบทใกล้เคียงกัน เพื่อป้องกันการพลัดตกหกล้มของผู้ป่วย ที่มีความพิการ
