Development of a Nursing Care Model to Prevent Crisis among Pregnant Women with Hypertension, Suratthani Hospital

Abstract

This research and development aimed to develop a model to prevent crisis in hypertensive   pregnancy and evaluate its effectiveness. The study was divided in 3 phases: 1) situation analysis, 2) development of model, and 3) the model's evaluation. The sample group consisted of: A) 40 professional nurses, selected using simple random sampling, and B) 50 pregnant women with   hypertension, selected using purposive sampling; they were divided into either control or experimental group (25 women/group). The research tools were: 1) the developed nursing model (IOC =1.00), 2) the nursing skills practice observation form (Inter rater =.90), 3) a knowledge measurement test (KR-20 = .78), and 4) a satisfaction assessment form (Cronbach's alpha coefficient = .93). Data were analyzed using descriptive statistics and Wilcoxon signed-rank test. The results were as follows.

1. The model developed is called GCC Model: 1) the letter G is for the nursing Guidelines for pregnant women with hypertension, 2) the C is for Clinical supervision, and 3) Competency.

2. After using the model, the incident rate of crisis among women with gestational hypertension was significantly lower (p<.05) than before. In terms of the service providers, their knowledge were significantly increasing (p<.01). Satisfaction was at the highest level (M= 3.60, S.D.= .51). All of them passed the criteria of the nursing skills.

Therefore, this nursing model should be used in hospitals with a similar context in order to prevent crisis in pregnant women with hypertension.
