การวิเคราะห์ต้นทุนกิจกรรมการพยาบาลผู้ป่วยสูงอายุผ่าตัดเปลี่ยนข้อเข่าเทียม ศูนย์ผ่าตัดฟื้นฟูสุขภาพ โรงพยาบาลทุติยภูมิแห่งหนึ่ง
บทคัดย่อ
การวิจัยเชิงพรรณนามีวัตถุประสงค์เพื่อวิเคราะห์ต้นทุนกิจกรรมการพยาบาลผู้ป่วยสูงอายุผ่าตัดเปลี่ยน ข้อเข่าเทียมแบบหนึ่งข้างและแบบสองข้างพร้อมกัน กลุ่มตัวอย่าง คือ รายงานต้นทุนทั้งหมดของบุคลากรพยาบาล จำนวน 14 คน และรายงานต้นทุนที่เกิดจากการให้บริการผู้ป่วยสูงอายุผ่าตัดเปลี่ยนข้อเข่าเทียม จำนวน 30 คน เลือกตัวอย่างแบบเฉพาะเจาะจง เครื่องมือวิจัยประกอบด้วยแบบบันทึกต้นทุนในมุมมองของผู้ให้บริการ จำนวน 3 ชุด ได้แก่ 1) แบบบันทึกข้อมูลต้นทุนต่อหน่วยบริการ ได้แก่ ค่าแรง ค่าวัสดุ ค่าครุภัณฑ์ โดยเก็บข้อมูลจากแหล่งข้อมูลทุติยภูมิของโรงพยาบาล 2) พจนานุกรมกิจกรรมการพยาบาลผู้ป่วยสูงอายุผ่าตัดเปลี่ยนข้อเข่าเทียม 3) แบบบันทึกข้อมูลต้นทุนกิจกรรมการพยาบาลผ่าตัด เก็บรวบรวมข้อมูลตั้งแต่เดือนพฤศจิกายน พ.ศ. 2565 ถึง เดือนมกราคม พ.ศ. 2566 ตรวจสอบคุณภาพเครื่องมือวิจัยกับผู้ทรงคุณวุฒิ 5 ท่าน ค่าความตรงเชิงเนื้อหา (CVI) เท่ากับ .97 และคำนวณค่าความเที่ยงสัมประสิทธิ์แอลฟาครอนบาค เท่ากับ .94 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา ได้แก่ ค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ผลการวิจัยพบว่า
1. ต้นทุนกิจกรรมพยาบาลรวมของผู้ป่วยสูงอายุผ่าตัดเปลี่ยนข้อเข่าเทียมแบบหนึ่งข้างและแบบสองข้าง พร้อมกัน เท่ากับ 74,480.47 บาท และ 134,838.68 บาท
2. ต้นทุนต่อกิจกรรมการพยาบาลเท่ากับ 2,53 บาท และ 2,466.56 บาท ตามลำดับ
3. ต้นทุนกิจกรรมการพยาบาลต่อรายเท่ากับ 7,448.04 บาท และ 7,154.04 บาท
ผลการวิจัยครั้งนี้ชี้ให้เห็นว่า การวิเคราะห์ต้นทุนกิจกรรมพยาบาลผู้ป่วยสูงอายุผ่าตัดเปลี่ยนข้อเข่าเทียม ผู้บริหารสามารถนำข้อมูลเชิงประจักษ์เรื่องขององค์ประกอบต้นทุน (ค่าแรง ค่าวัสดุ ค่าลงทุน) เพื่อใช้ประกอบการ วางแผนการบริหารต้นทุนและงบประมาณได้สอดคล้องกับกิจกรรมการผ่าตัด ทำให้มีการเตรียมทรัพยากรด้านต้นทุนได้อย่างเพียงพอ ส่งผลให้การจัดบริการเป็นไปตามเป้าหมาย
