การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษากระบวนทัศน์ของผู้สูงอายุและครอบครัวในการเตรียมความ

พร้อมสู่การตายอย่างสงบของผู้สูงอายุที่โรงพยาบาล ใช้วิธีการวิจัยแบบผสมผสาน คือ การวิจัยเชิงปริมาณและเชิงคุณภาพ สุ่มตัวอย่างแบบแบ่งชั้นจากกลุ่มผู้สูงอายุที่มารับบริการในสถานพยาบาลในภาคตะวันออกและญาติหรือผู้ดูแล ได้กลุ่มตัวอย่าง จำนวน 384 คน (ผู้สูงอายุ จำนวน 77 คน ญาติหรือผู้ดูแล 307 คน) ใช้สถิติค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน ค่าสัมประสิทธิ์สหสัมพันธ์ของสเปียร์แมนและการวิเคราะห์เนื้อหา

ผลการศึกษาวิจัย พบว่า ความต้องการการดูแลในกลุ่มผู้สูงอายุ และผู้ดูแลในครอบครัว ในภาพรวม พบว่า อยู่ในระดับมาก ( =3.61, SD = 0.72) ตัวแปรคุณลักษณะของผู้สูงอายุที่มีความสัมพันธ์ทางบวก ในระดับสูงกับความต้องการการดูแล คือ ระดับการศึกษา (rs =0.65, p < 0.01) และ เพศ (rs=0.49, p < 0.05) ส่วนตัวแปรคุณลักษณะของผู้สูงอายุที่มีความสัมพันธ์ทางลบ ในระดับสูงกับความต้องการการดูแล คือ โรคประจำตัว (rs=-0.50, p < 0.05) ตัวแปรคุณลักษณะของผู้ดูแลที่มีความสัมพันธ์ทางบวกในระดับสูงกับความต้องการการดูแล คือ ระดับการศึกษา (rs=0. 50, p < 0.05) ตัวแปรคุณลักษณะของผู้ดูแลที่มีความสัมพันธ์ทางลบ ในระดับสูงกับความต้องการการดูแล คือ ศาสนา (rs=-0.66, p < 0.01) อายุ, เพศ , อาชีพและรายได้ (rs=-0.52, -0.57,-0.59,-0.62, p < 0.05)   ผลการวิเคราะห์เชิงคุณภาพ พบว่า ในทัศนะของผู้สูงอายุ การตายอย่างสงบที่โรงพยาบาล เป็นการตายที่ไม่เจ็บปวด ไม่ทุกข์ทรมาน โดยได้จัดการสิ่งต่าง ๆ ที่คั่งค้างไว้ให้สำเร็จลงแล้ว และแวดล้อมด้วยผู้เป็นที่รัก ไม่ถูกทอดทิ้งอย่างโดดเดี่ยวเดียวดาย เป็นการตายที่เชื่อมั่นได้ว่าได้รับการช่วยเหลืออย่างดีที่สุดแล้ว เพื่อให้มีชีวิตรอด  โดยญาติและผู้ดูแลในครอบครัวจะได้รับการช่วยเหลือดูแลหลังที่ตนเสียชีวิตแล้วจากแพทย์ พยาบาลซึ่งเป็นผู้ที่มีความรู้ มีจรรยาบรรณวิชาชีพ  สภาพของผู้เสียชีวิตจะได้รับการปกป้องดูแลอย่างสมศักดิ์ศรีความเป็นมนุษย์ และต้องการให้เพิ่มความใกล้ชิด เห็นอกเห็นใจ ให้ความยืดหยุ่นแก่ญาติและผู้ป่วยระยะสุดท้ายโดยให้ความดูแลอย่างเอื้ออาทร และผ่อนปรนกฎระเบียบลงบ้าง

ABSTRACT[1]

Mix method research was used to explore the elderly and family’s paradigm for

preparing the end-of-life on hospital based care. Three hundred eighty four samples included 77 older persons and 307 family members in Eastern region of Thailand. Date was collected using care need questionnaires. Statistics employed by using frequency, percentage, mean, standard deviation, the Spearman Rank correlation coefficient and content analysis. 

The study revealed that the end-of-life care needs were in the high level (  = 3.61 , SD = 0.72). The relationship between characteristics of the elderly and end-of-life need in positive direction at high level were education (rs =0.65, p < 0.01) and gender (rs = 0.49,

p < 0.05 and  the relationship between characteristics of the elderly and end-of-life need in negative direction at high level was underlying of chronic diseases  (rs = -0.50, p < 0.05). The relationship between characteristics of the caregivers in elder’s family and end-of-life need in positive direction at high level was education (rs =   0. 50, p < 0.05). The relationship between characteristics of the caregivers in elder’s family and end-of-life need in negative direction at high level were religion (rs = -0.66, p < 0.01), age, gender, occupation and income (rs =-0.52, -0.57,-0.59,-0.62, p < 0.05). Summarized of the in-depth interviews found that; Perception of the elderly in peaceful end-of-life care at hospital means die with painless, do not suffering and can do anything that they have to do already .Surrounding with people who love. Do not be neglected alone. Death at hospital was a sentiment that has been optimally help from the medical doctor to have survived. Relatives and family members assure to get help and care from medical personnel with professional ethics after they die. Condition of deceased will be completely protected human dignity. Wish to increase the intimacy to the relatives and family members of the elderly who pass away and flexibility for providing caring or lenient the rules of hospital.
