บทคัดย่อ
งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลกระทบ ความท้าทาย อุปสรรคในการป้องกันควบคุมโรคและภัยสุขภาพภายหลังการถ่ายโอนโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ไปสังกัดองค์การบริหารส่วนจังหวัด (อบจ.) และจัดทำข้อเสนอเชิงนโยบายในบริบทของเขตสุขภาพที่ 4 ใช้ระเบียบวิธีวิจัยแบบผสมวิธี รวบรวมข้อมูลจาก การสัมภาษณ์เชิงลึก สนทนากลุ่ม ประชุมกลุ่มและการสอบถาม จากผู้มีส่วนได้ส่วนเสียในระดับจังหวัด อำเภอและตำบล จำนวน 170 คน วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการตีความและสร้างข้อสรุป วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา นำเสนอเป็นค่าความถี่ ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน พบว่า โรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอน มีความคล่องตัวในการบริหารงบประมาณ วัสดุอุปกรณ์และบุคลากร มากขึ้น พบปัญหานโยบายไม่ต่อเนื่องระหว่างส่วนกลางและท้องถิ่น อบจ.ไม่มีบุคลากรที่มีความเชี่ยวชาญด้านการเฝ้าระวัง ป้องกันควบคุมโรค อบจ.ยังไม่มีแผนระยะยาวในด้านการเฝ้าระวัง ป้องกันควบคุมโรคและการพัฒนาบุคลากร ไม่มีกรอบการประเมินผลการดำเนินงานป้องกันควบคุมโรคที่ชัดเจน สรุปข้อเสนอเชิงนโยบาย ดังนี้ 1) พัฒนาและเชื่อมต่อระบบฐานข้อมูลเฝ้าระวังโรคและภัยสุขภาพ 2) พัฒนาทรัพยากรบุคคล อบจ.และ รพ.สต.ถ่ายโอน ด้านการเฝ้าระวัง ป้องกันควบคุมโรคและการจัดการภาวะฉุกเฉินกรณีโรคระบาด 3) ส่งเสริมการป้องกันควบคุมโรคและภัยสุขภาพเชิงรุก 4) พัฒนาระบบการตอบโต้ภาวะฉุกเฉินทางด้านสาธารณสุข ของ อบจ. 5) สนับสนุนทรัพยากรเพื่อการป้องกันควบคุมโรค อย่างเพียงพอและทันเวลา ทั้งนี้ได้นำไปทดลองใช้ในพื้นที่แล้วสามารถปฏิบัติได้จริง ประชาชนมากกว่าร้อยละ 80 มีความพึงพอใจด้านการเข้าถึงบริการสุขภาพ การป้องกันควบคุมโรคและภัยสุขภาพและการให้บริการของ รพ.สต. ผู้มีส่วนได้ส่วนเสียมากกว่าร้อยละ 80 เห็นว่านโยบายที่เสนอ ช่วยเสริมสร้างความร่วมมือในการป้องกันควบคุมโรคและภัยสุขภาพ สามารถนำไปพัฒนาและตอบสนองต่อปัญหาด้านสุขภาพของประชาชนในพื้นที่ได้ดียิ่งขึ้น
