Development of a Health Behavior Modification Model Using the PKR Model for At-Risk Groups of Diabetes and Hypertension in Phrom Khiri District, Nakhon Si Thammarat Province

Abstract
Background: Diabetes mellitus and hypertension are major global public health problems. These diseases adversely affect quality of life and may lead to severe complications. Studies conducted in Thailand have shown that disease control rates remain below established standards.
Objectives: To develop and evaluate the effectiveness of the PKR Model, a health behavior modification model for individuals at risk of diabetes and hypertension in Phrom Khiri District, Nakhon Si Thammarat Province.
Methods: This action research used the Planning–Action–Observation–Reflection (PAOR) process to develop the PKR Model. The sample comprised 92 participants, equally assigned to an experimental group and a control group, with 46 participants in each group. The experimental group received care based on the PKR Model, which included collaborative goal setting, knowledge provision, and continuous reinforcement, while the control group received standard care. Data were collected using questionnaires, in-depth interviews, and measurements of health indicators. The data were analyzed using paired-samples t-tests and independent-samples t-tests.
Results: Participants receiving the PKR Model demonstrated statistically significant improvements in health indicators (p<0.001). Mean blood glucose levels decreased by 17.89 mg/dL, systolic blood pressure decreased by 7.61 mmHg, diastolic blood pressure decreased by 11.44 mmHg, and waist circumference decreased by 2.58 cm. Participants also reported a high level of satisfaction with the model.
Conclusion: The PKR Model was effective in promoting self-management and improving health indicators among individuals at risk of diabetes and hypertension. The model was appropriate for the community context and may be adapted for implementation in other settings or among populations at risk of other chronic diseases.
