การศึกษาประสิทธิผลการรักษาโรคข้อเข่าเสื่อมด้วยการฝังเข็มร่วมกับการนวดทุยหนา ในผู้สูงอายุ เขตอำเภอบางพลี จังหวัดสมุทรปราการ

บทคัดย่อ

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาประสิทธิผลของการฝังเข็มและการฝังเข็มร่วมกับการนวดทุยหนาในผู้สูงอายุที่เป็นโรคข้อเข่าเสื่อมที่อาศัยอยู่ในอำเภอบางพลี จังหวัดสมุทรปราการ จำนวน 61 ราย จากนั้นใช้โปรแกรม SPSS 21.0 แบ่งผู้เข้าร่วมวิจัยออกเป็น 2 กลุ่ม ได้แก่ กลุ่มควบคุม 30 ราย และกลุ่มทดลอง 31 ราย ด้วยวิธีการสุ่ม กลุ่มควบคุมรักษาด้วยการฝังเข็มเพียงสัปดาห์ละครั้ง ครั้งละ 20 นาที กลุ่มทดลองรักษาด้วยการฝังเข็ม 20 นาที และนวดทุยหนา 10-15 นาที สัปดาห์ละครั้ง ระยะเวลาการรักษาของทั้งสองกลุ่มคือ 8 สัปดาห์ และได้รับการประเมินด้วยแบบสอบถาม Western Ontario และ McMaster Universities Arthritis Index (WOMAC) ก่อนการรักษา (0), 3, 5 และ 8 สัปดาห์หลังการรักษา และวิเคราะห์ผล WOMAC ด้วยค่า t-test ผลการศึกษา พบว่ากลุ่มทดลองมีอายุเฉลี่ย 66.09 ปี ค่าดัชนีมวลกาย (BMI) 25.93 กก./ม.² ระยะเวลาของอาการปวดอยู่ที่ 4.64 ปี กลุ่มควบคุมมีอายุเฉลี่ย 64.86 ปี ค่าดัชนีมวลกาย 26.48 กก./ม.²และระยะเวลาแสดงอาการปวดอยู่ที่ 4.00 ปี ทั้งสองกลุ่มมีอายุเฉลี่ย ค่าดัชนีมวลกายและระยะเวลาของอาการไม่แตกต่างกัน (P>0.05) การเปรียบเทียบก่อนการรักษาของทั้งสองกลุ่มกับหลังการรักษา พบว่าหลังการรักษา อาการปวดลดลงและการทำงานของข้อดีขึ้น (P<0.001) การเปรียบเทียบระหว่างกลุ่มทดลองและกลุ่มควบคุมหลังรับการรักษา พบว่า คะแนนอาการปวดมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<0.001) กลุ่มทดลองมีความสามารถในการทำงานของข้อเข่าดีขึ้นกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (P<0.001) แต่ไม่มีความแตกต่างอย่างมีนัยสำคัญทางสถิติกับอาการข้อฝืดข้อยึด (P>0.05) ผลการศึกษาโดยรวม พบว่า คะแนน WOMAC ในกลุ่มทดลองลดลงอย่างมีนัยสำคัญ (P<0.001) เมื่อเปรียบเทียบกับกลุ่มควบคุม สรุปการฝังเข็มร่วมกับการนวดทุยหนาในการรักษาโรคข้อเข่าเสื่อมเป็นทางเลือกในการรักษาที่มีประสิทธิผลและให้ผลการรักษาที่ดีกว่าการฝังเข็มเพียงอย่างเดียว
