ปัจจัยที่มีอิทธิพลต่อภาวะเลือดออกหลังผ่าตัดในผู้ป่วยที่ได้รับการผ่าตัดหัวใจแบบเปิด

งานวิจัยเชิงบรรยายนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีอิทธิพลต่อภาวะเลือดออกหลังผ่าตัดในผู้ป่วยที่ได้รับการผ่าตัดหัวใจแบบเปิดในโรงพยาบาลเอกชนแห่งหนึ่ง ในช่วงปี พ.ศ. 2558-2559 จำนวน 245 ราย วิธีการศึกษาใช้การศึกษาแบบย้อนหลัง โดยเก็บข้อมูลย้อนหลังจากเวชระเบียนผู้ป่วยที่ได้รับการผ่าตัดหัวใจแบบเปิดและมีอายุมากกว่า 18 ปี วิเคราะห์ข้อมูลด้วยสถิติบรรยาย สถิติไคสแคว์ สถิติฟิชเชอร์ และวิเคราะห์การถดถอยโลจิสติคทวิภาค
ผลการศึกษา พบว่า ตัวแปรที่มีความสัมพันธ์กับภาวะเลือดออกหลังผ่าตัดในผู้ป่วยที่ได้รับการผ่าตัดหัวใจแบบเปิดอย่างมีนัยสำคัญทางสถิติ ได้แก่ โรคเบาหวาน สัดส่วนการบีบตัวของหัวใจห้องล่างซ้าย ค่าการแข็งตัวของเลือดหลังผ่าตัด ส่วนปัจจัยที่มีอิทธิพลต่อภาวะเลือดออกหลังผ่าตัดหัวใจแบบเปิด คือ สัดส่วนการบีบตัวของหัวใจห้องล่างซ้าย (AOR= 12.53, 95%CI = 1.35-116.58, p=0.03) และค่าการแข็งตัวของเลือดหลังผ่าตัด (AOR= 7.17, 95%CI = 1.15-44.71, p=0.04) (Nagelkerke R2 = 0.34)
ผลการวิจัยเสนอแนะว่า พยาบาลและทีมสุขภาพควรมีการตรวจและติดตามผลการตรวจสัดส่วนการบีบตัวของหัวใจห้องล่างซ้ายก่อนผ่าตัดหัวใจแบบเปิดและค่าการแข็งตัวของเลือดหลังผ่าตัด และควรสร้างแนวปฏิบัติเพื่อเฝ้าระวังและป้องกันภาวะเลือดออกรุนแรงก่อนและหลังผ่าตัดหัวใจแบบเปิด
