This quasi-experimental study with a two-group pretest-posttest design aimed to examine the effects of a health literacy program on individuals at risk of knee osteoarthritis in an urban pilot community. Participants were 40 adults aged 40 years and older who met the inclusion criteria and were randomly assigned to either the experimental group (n = 20) or the control group (n = 20). The intervention, lasting seven weeks, comprised six components: access to health information and services, knowledge, communication skills, self-management skills, decision-making skills, and media literacy. Data were collected using four instruments: a demographic questionnaire, the Oxford Knee Score for disease severity, a health literacy assessment for knee osteoarthritis prevention, and a health behavior assessment for disease prevention. The content validity index of the health literacy and health behavior instruments were 0.72 and 0.99, respectively, with reliability of 0.80. Data were analyzed using descriptive statistics (frequency, percentage, mean, standard deviation) and inferential statistics (independent t-test for between-group comparison and paired t-test for within-group comparison). The results indicated that after the program, the experimental group showed significantly higher health behavior scores and median health literacy for knee osteoarthritis prevention than both their pretest scores and the control group (t = 6.860, -3.539; p < .001 and t = 3.286, -4.078; p < .002, respectively). These findings suggest that the program can be effectively applied to promote self-care and prevent knee osteoarthritis among middle-aged adults in the community.
