บทคัดย่อ

บทนำ หมู่เลือดอาร์เอชเดล (RhDel) เป็นหมู่เลือด RhD บวก (RhD positive) ที่มีการแสดงออกของแอนติเจน D ระดับต่ำมากจนไม่สามารถตรวจพบได้ด้วยวิธีซีโรโลยีมาตรฐาน ส่งผลให้อาจถูกจัดเป็น RhD ลบ (RhD negative)  จำเป็นต้องทดสอบเพิ่มเติมด้วยเทคนิค adsorption-elution หรือเทคนิคอณูชีววิทยาเพื่อแยก RhDel ออกจาก RhD ลบแท้จริง หมู่เลือด RhDel ชนิดเอเชีย (Asian-type DEL) พบได้บ่อยในประชากรเอเชีย แม้ว่ามีการแสดงออกแอนติเจน D ต่ำ แต่ยังสามารถกระตุ้นการสร้าง anti-D ในผู้ป่วย RhD ลบได้ อาจนำไปสู่ภาวะเม็ดเลือดแดงแตกหลังการรับเลือด (Hemolytic transfusion reaction; HTR) หรือภาวะเม็ดเลือดแดงแตกของทารกในครรภ์และทารกแรกเกิด (hemolytic disease of the fetus and newborn; HDFN) ดังนั้น ความเข้าใจเกี่ยวกับความชุก ความสำคัญทางคลินิก และการตรวจทางห้องปฏิบัติการของหมู่เลือด Asian-type DEL จึงมีความสำคัญต่อความปลอดภัยของการให้และรับเลือด

วัตถุประสงค์ เพื่อทบทวนและวิเคราะห์ข้อมูลเกี่ยวกับความชุกในกลุ่มประชากร ความสำคัญทางคลินิกและแนวทางการให้เลือดอย่างปลอดภัยของหมู่เลือด Asian-type DEL

วิธีการศึกษา ทบทวนวรรณกรรมอย่างเป็นระบบจากฐานข้อมูล PubMed และ Google Scholar ครอบคลุมช่วงเวลาการตีพิมพ์ปี ค.ศ. 2000 – 2025

ผลการศึกษา Asian-type DEL เป็น RhDel ที่มีความชุกสูงที่สุดในผู้ที่มีหมู่เลือด RhD ลบในเอเชียตะวันออก เอเชียตะวันออกเฉียงใต้ และประเทศไทย หลักฐานจากการศึกษาพบว่า ผู้ที่มีหมู่เลือด Asian-type DEL ไม่สร้าง anti-D ภายหลังได้รับเลือด RhD บวก หรือตั้งครรภ์ทารก RhD บวก ในทางตรงกันข้ามเลือดบริจาค Asian-type DEL สามารถกระตุ้นสร้าง anti-D ในผู้ป่วย RhD ลบที่แท้จริงได้

สรุป Asian-type DEL มีความสำคัญต่อการให้และรับเลือดในประชากรเอเชีย การตรวจระบุหมู่เลือดอย่างถูกต้องโดยใช้เทคนิคอณูชีววิทยาควบคู่กับเทคนิคทางซีโรโลยี จึงเป็นแนวทางที่เหมาะสมและผู้ป่วย Asian-type DEL สามารถรับเลือด RhD บวกได้อย่างปลอดภัย ในขณะที่โลหิตบริจาค Asian-type DEL ควรได้รับการจัดการเหมือนเลือด RhD บวกและจำกัดการให้เฉพาะผู้ป่วย RhD บวกเท่านั้น
