บทคัดย่อ

บทนำ โรคไซนัสอักเสบจากเชื้อราลุกลามแบบเฉียบพลันชนิด
mucormycosis พบอัตราป่วยตายสูงถึงร้อยละ 23.07 มีการลุกลามเร็วไปยังอวัยวะข้างเคียง เช่น ลูกตา สมอง อาการระยะแรกมักไม่จำเพาะ จึงมักได้รับการวินิจฉัยล่าช้าส่งผลให้อัตราการเกิดความพิการและเสียชีวิตสูงมาก

วัตถุประสงค์ เพื่อศึกษาลักษณะทางประชากรศาสตร์ อาการ โรคประจำตัวที่เป็นปัจจัยเสี่ยง การรักษาการดำเนินโรคระยะเวลานอนโรงพยาบาล และภาวะแทรกซ้อนของกลุ่มตัวอย่างที่ได้รับการวินิจฉัยเป็นโรคไซนัสอักเสบจากเชื้อราลุกลามแบบเฉียบพลันจาก mucormycosis ในโรงพยาบาลพุทธโสธร

วิธีการศึกษา รายงานชุดกรณีศึกษา โดยศึกษาตั้งแต่ปี พ.ศ. 2560-2568 เนื่องจากโรคนี้มีอุบัติการณ์ต่ำจึงรวบรวมข้อมูลผู้ป่วยทั้งหมดที่มีในฐานข้อมูล จำนวน 6 ราย

ผลการศึกษา กลุ่มตัวอย่างทั้งหมดเป็นเพศชาย อายุเฉลี่ยเท่ากับ 55 (2.22) ปี เป็นเบาหวานมากที่สุด ร้อยละ 66.67 ติดเชื้อรา mucorales อย่างเดียวร้อยละ 83.33 โดยไซนัสที่พบมากที่สุดคือ ethmoid sinus ร่วมกับ maxillary sinus ร้อยละ 83.33 ค่ามัธยฐานก่อนเริ่มรักษา 10 (6.25-12.25) วัน อาการนำพบอาการทางตา มากที่สุด ร้อยละ 50 อวัยวะข้างเคียงที่เชื้อราลุกลามไปมากสุดคือลูกตา หรือเนื้อเยื่อรอบลูกตา ทุกรายได้รับการรักษาด้วยยา amphotericin B ร่วมกับการผ่าตัดผ่าไซนัสและอวัยวะที่เชื้อราลุกลาม โดยค่ามัธยฐานของระยะเวลาที่ได้ยาฆ่าเชื้อรา 52 (41.25-107.00) วัน พบอัตราการเสียชีวิต ร้อยละ 16.67

สรุป โรคไซนัสอักเสบจากเชื้อราลุกลามแบบเฉียบพลันชนิด mucormycosis ในโรงพยาบาลพุทธโสธรส่วนมากมีโรคประจำตัวเป็นเบาหวาน มีอาการนำ คือ อาการทางตา มักพบรอยโรคที่ ethmoid sinus และ maxillary sinus มีการลุกลามไปลูกตา และเนื้อเยื่อบริเวณรอบลูกตามากที่สุด แม้ทำการรักษาตามมาตรฐานทั้งการผ่าตัดและยาฆ่าเชื้อรา โอกาสเสียชีวิตพบได้ร้อยละ 16.67
