The Development of a Continuity of Care Program Using Transitional Care Model among Family Caregivers of Patients with Chronic Non-communicable Disease Experiencing Functional Impairment in Phrapokklao Hospital, Chanthaburi Province: A Pilot Study

Abstract

This research and development aimed to develop and study the effects of a continuity of care program using transitional care model among family caregivers of patients with chronic non-communicable disease (NCD) experiencing functional impairment. This research consisted of 5 phases: phase 1 is studying caregiving situation (57 family caregivers at intermediate wards 1, 2, and 3 of Phrapokklao Hospital), phase 2 is developing a continuity of care program using transitional care model, phase 3 is testing feasibility of a continuity of care program with 2 family caregivers, phase 4 is revising a continuity of care program, and phase 5 is evaluating a continuity of care program with family caregivers at intermediate wards 3 of Phrapokklao Hospital (15 experimental and 15 control groups). The research instruments included a continuity of care program using transitional care model, a handbook of caregiving NCD patients, a mockup model, a home simulation, the Six Item Cognitive Impairment Test (6CIT), a demographic questionnaire, a caregiving situation questionnaire, and an assessment form of caregiving skills with reliabilities in the range of .75–.82. The implementation and data collection were conducted from February, 2021 to February, 2022. Data were analyzed by frequency, percentage, mean, standard deviation, Wilcoxon signed-rank test, and Mann-Whitney U Test.

The research results revealed that 1) a continuity of care program using transitional care model consisted of giving knowledge regarding diseases and training caregiving skills; 2) after receiving the program, the experimental group had statistically significantly higher mean scores of each caregiving skill than those of before receiving the program (p < .01); and 3) the experimental group after receiving the program had statistically significantly higher mean scores of each caregiving skill than those of the control group after receiving the routine nursing care (p < .01).

This research suggests that nurses should train caregiving skills to family caregivers by using instrument and home simulation in order to increase self-confidence of caregiving skills.
