การแปลงแนวทางเวชปฏิบัติโรคไตเรื้อรังสู่การดูแลในระบบบริการสุขภาพปฐมภูมิ: ข้อเสนอรูปแบบการดูแลที่นำโดยพยาบาลเวชปฏิบัติชุมชนในยุคดิจิทัล

โรคไตเรื้อรังเป็นปัญหาสาธารณสุขสำคัญที่มีแนวโน้มเพิ่มขึ้นอย่างต่อเนื่อง และสัมพันธ์กับภาวะแทรกซ้อนรุนแรง คุณภาพชีวิตที่ลดลง รวมถึงภาระค่าใช้จ่ายของระบบสุขภาพ แม้ประเทศไทยจะมีแนวทางเวชปฏิบัติสำหรับการคัดกรอง การชะลอการดำเนินโรค และการส่งต่อรักษาที่ชัดเจน แต่ยังคงมีความท้าทายในการนำข้อเสนอแนะดังกล่าวไปสู่การปฏิบัติจริงในระบบบริการสุขภาพปฐมภูมิและชุมชน บทความวิชาการนี้มีวัตถุประสงค์เพื่อสังเคราะห์แนวทางเวชปฏิบัติโรคไตเรื้อรังที่สำคัญ วิเคราะห์ความท้าทายของการดูแลในระบบบริการสุขภาพปฐมภูมิ และเสนอรูปแบบการดูแลผู้ป่วยโรคไตเรื้อรังในชุมชนที่นำโดยพยาบาลเวชปฏิบัติชุมชนร่วมกับเทคโนโลยีสุขภาพดิจิทัล โดยสังเคราะห์หลักฐานจากแนวทางเวชปฏิบัติของประเทศไทย แนวทางเวชปฏิบัติทางคลินิกสำหรับการประเมินและการจัดการโรคไตเรื้อรัง ค.ศ. 2024 ของ Kidney Disease: Improving Global Outcomes (KDIGO) แนวคิดการดูแลโรคเรื้อรัง การสนับสนุนการดูแลตนเอง และหลักฐานด้านเทคโนโลยีสุขภาพดิจิทัลทางการพยาบาล จนนำไปสู่ข้อเสนอรูปแบบการดูแลผู้ป่วยโรคไตเรื้อรังในชุมชนที่นำโดยพยาบาลเวชปฏิบัติชุมชน ซึ่งประกอบด้วย 5 องค์ประกอบ ได้แก่ 1) การค้นหากลุ่มเสี่ยงเชิงรุก 2) การจัดระดับความเสี่ยง 3) การฝึกสอนเพื่อส่งเสริมการดูแลตนเอง 4) การติดตามต่อเนื่องผ่านชุมชนและเทคโนโลยีสุขภาพดิจิทัล และ 5) การประสานการส่งต่ออย่างเป็นระบบ รูปแบบการดูแลดังกล่าวมีศักยภาพในการสนับสนุนการนำแนวทางเวชปฏิบัติไปสู่การปฏิบัติจริง เพิ่มความต่อเนื่องของการดูแลในระบบสุขภาพไทย อย่างไรก็ตาม รูปแบบดังกล่าวยังเป็นข้อเสนอเชิงแนวคิด จึงจำเป็นต้องมีการศึกษานำร่องเพื่อประเมินความเป็นไปได้ ผลลัพธ์ทางคลินิก และความคุ้มค่าทางเศรษฐศาสตร์สุขภาพในบริบทการใช้งานจริงต่อไป
