การประเมินสุขภาพผู้สูงอายุในชุมชน: บทบาทของพยาบาลเวชปฏิบัติ

เมื่อเข้าสู่วัยสูงอายุ ผู้สูงอายุมักจะมีปัญหาด้านสุขภาพทำให้ต้องได้รับการดูแลในหลากหลายมิติ ดังนั้นการคัดกรองค้นหาผู้สูงอายุที่มีความเสี่ยงต่อการเกิดโรคต่าง ๆ เช่นภาวะซึมเศร้า สมองเสื่อม หรือ การเดินและการทรงตัวผิดปกติจึงมีความสำคัญและจำเป็นเพื่อให้ผู้สูงอายุได้รับการดูแลที่เหมาะสมตามความต้องการด้านสุขภาพ พยาบาลเวชปฏิบัติมีบทบาทสำคัญในการค้นหาผู้สูงอายุที่มีปัญหาสุขภาพในชุมชนโดยการตรวจคัดกรองตามระบบและประเมินสุขภาพผู้สูงอายุอย่างเป็นองค์รวม โดยคำนึงถึงลักษณะพิเศษของผู้สูงอายุที่แตกต่างจากวัยอื่น และเน้นการประเมินสุขภาพ 3 ด้าน คือ 1) สุขภาพกาย และ สมรรถนะทางกาย 2) สุขภาพจิตใจและการรู้คิด และ 3) สังคมและสิ่งแวดล้อม โดยสหสาขาวิชาชีพช่วยค้นหาปัญหาที่ส่งผลต่อภาวะสุขภาพของผู้สูงอายุ พร้อมทั้งวางแผนการดูแลตามความต้องการด้านสุขภาพเพื่อคงไว้ซึ่งความสามารถในการดำเนินกิจวัตรประจำวัน ลดภาวะพึ่งพา วัตถุประสงค์ของบทความนี้เพื่ออธิบายหลักการประเมินสุขภาพผู้สูงอายุในด้านต่าง ๆ เพื่อนำไปสู่การประยุกต์ใช้การประเมินสุขภาพผู้สูงอายุในชุมชน
