Comprehensive Geriatric Assessment in Community: Roles of Nurse Practitioner

As people age, they often have complex health problems requiring multidimensional care. It is essential to identify older adults at risk for a particular condition such as depression, dementia, and gait abnormality so they will receive the care they need. Nurse practitioners play an important role in identifying older adults with health problems in the community through systematically screens and comprehensive geriatric assessment (CGA) by concerning the unique characteristics of older adults that are different from other ages and focusing on health assessment in 3 domains: 1) physical and functional health, 2) psychological health and cognitive, and 3) social and environment. The multidisciplinary approach helps find problem lists that affect the health of the older adults, leading to the development of an individual care plan to meet those needs to maintain activities of daily living and dependency. This article aims to explain how to apply CGA in community settings to address each domain of aging.
