Abstract

This quasi-experimental research with a two-group pretest–posttest design aimed to examine the effects of an intensive health behavior modification program on knowledge, health behaviors, glycated hemoglobin (HbA1c) levels, and remission among patients with type 2 diabetes mellitus in the Maharaj Nakhon Si Thammarat Hospital Network. The sample consisted of 70 patients with type 2 diabetes mellitus diagnosed within the past five years. Participants were selected through purposive sampling and matched by sex, age, and educational level before being assigned to either the experimental group or the control group, with 35 participants in each group. Data were collected using a knowledge questionnaire with a Kuder–Richardson 21 reliability coefficient of .83 and a health behavior questionnaire with a Cronbach’s alpha coefficient of .82. Data were analyzed using descriptive statistics and t-tests. The results revealed that:

1. After participating in the program, patients in the experimental group had significantly higher mean scores for knowledge and health behaviors than before the intervention (t = 27.81, 34.90; p < .001) and significantly higher than those in the control group (t = 29.53, 67.63; p < .001). In addition, HbA1c levels in the experimental group were significantly lower than before participation and lower than those in the control group at the .001 significance level (t = 24.88, 14.38; p < .001).

2. Following participation in the program, 57.14% of patients in the experimental group achieved diabetes remission, 22.85% were able to discontinue medication, and 17.14% were able to reduce medication dosage.

The findings suggest that diabetes clinic personnel may apply this program through multidisciplinary care and continuous follow-up to promote health behavior modification, improve glycemic control, and increase the likelihood of diabetes remission among patients with type 2 diabetes mellitus.
