The Developing a Tuberculosis Care Model Using Directly Observed Treatment Combined with Electronic Medication Adherence at Rangae Hospital, Narathiwat Province

Abstract

This research and development (R&D) study was conducted in three phases: 1) examining the current situation of tuberculosis (TB) medication adherence care through a systematic literature review; 2) developing a tuberculosis patient care model incorporating treatment supporters for Directly Observed Therapy (DOT) combined with Video Observed Treatment (VOT) through electronic communication technology, and 3) evaluating the effectiveness of the developed model using a one-group pretest–posttest quasi-experimental design. The sample consisted of 45 patients with tuberculosis. Data were collected using a demographic questionnaire, a tuberculosis literacy questionnaire with a Kuder–Richardson Formula 20 (KR-20) reliability coefficient of .82, and a satisfaction questionnaire regarding the developed care model. Data were analyzed using descriptive statistics and paired t-tests.


The findings revealed that, after receiving the intervention, the participants demonstrated a high level of tuberculosis literacy (M = 17.09, SD = 1.25), which was significantly higher than their pre-intervention score (t = 20.48, p < .001). Participants also reported a high level of satisfaction with the developed care model (M = 4.09, SD = .62).


The developed tuberculosis care model, which integrates treatment supporters for directly observed therapy with electronic medication observation, can be applied by professional nurses, public health personnel, village health volunteers, and healthcare facilities in Thailand’s southern border provinces as a practical approach to improving tuberculosis care and promoting better health outcomes among patients with tuberculosis.
