Development of Nursing Care Services for Patients with Hypertension Crisis in Pregnancy, Maternity Department, Phra Nakhon Si Ayutthaya Hospital

Abstract
This action research aimed to enhance the nursing care service system for managing hypertension crisis among women delivering at Phranakhon Si Ayutthaya Hospital. The study proceeded in three phases: 1) situational analysis of the nursing care service for hypertension crisis in pregnancy management, 2) development of nursing guidelines, and 3) evaluation of outcomes using a quasi-experimental design with a targeted sample of 30 obstetric nurses and 30 pregnant women with hypertension crisis across Phranakhon Si Ayutthaya Hospital and its affiliated network hospitals.
Results revealed the components of the maternity care service system, including crisis management protocols, risk assessment methods, magnesium sulfate administration guidelines, seamless referral systems, and nurse competency development through coaching approaches. Implementation of this system led to a statistically significant increase in nurses' knowledge and competencies (p < .01) without undesirable incidents such as unanticipated transfers, unplanned admissions to intensive care units, or uncoordinated resuscitation efforts.
The development of nursing care services for patients with hypertension crisis in pregnancy necessitates the cultivation of nurse competencies through coaching approaches. This ensures that nurses with diverse work experiences can confidently deliver quality care, thus reducing undesired incidents and risks.
