Timing of Antibiotic Prophylaxis Administration for Total knee Arthroplasty In Songkhla Hospital

Abstract

This descriptive study purposed to 1) study the duration of administration of preoperative antibiotic for replacement of knee osteoarthritis in Songkhla Hospital, and 2) to study the rationales for administering preoperative antibiotic treatment to patients with knee arthritis. The research was conducted in 2 steps. Step 1. was to study the timing of antibiotic prophylaxis administration for total knee Arthroplasty. A sample was the patients who were treated with antibiotics (Cefazolin) prior to knee replacement surgery for osteoarthritis in Songkhla Hospital. The research instrument was a record of the duration of administration of antibiotics prior to surgery. The IOC value was 1.00. Data were analyzed by descriptive statistics. Step 2 focused on the reasons for antibiotic prophylaxis administration for total knee Arthroplasty. In this step, 17 anesthesia nurse who working in the anesthesia nursing department in Songkhla Hospital were purposively recruited. They were interviewed by using a semi-structured open-ended questionnaire and the received information were analyzed by content analysis.

1. Patients undergoing anesthesia under general anesthesia had a preoperative period of 30-60 minutes before surgery 30.23 % (µ =32.82 minutes, σ = 3.41). For less than 30 minutes prior to surgery 69.67% (µ =22.33 minutes, σ = 3.14) was found in patients undergoing under spinal anesthesia. Duration of administration of antibiotics before surgery was at 30-60 minutes and the average length of surgery was 50 % (µ = 32.44 minutes, σ = 4.33) for less than 30 minutes before the operation 62. 50%. (µ = 19.40 minutes, σ = 6.65).

2. Rationales for administering antibiotics for ankle replacement and knee osteoarthritis patients in Songkhla Hospital include patient safety from drug sensitivity and to maximize drug use in preventing infection, surgery.
