ปัจจัยที่เกี่ยวข้องกับการตัดสินใจเลือกวิธีบำบัดทดแทนไตของผู้ป่วยโรคไตเรื้อรังระยะที่ 4 และ 5: การทบทวนวรรณกรรมอย่างเป็นระบบแบบผสมผสาน

การตัดสินใจเลือกวิธีบำบัดทดแทนไตในผู้ป่วยโรคไตเรื้อรังระยะที่ 4 และ 5 เป็นกระบวนการที่ซับซ้อนและส่งผลต่อคุณภาพชีวิตในระยะยาว ปัจจัยที่มีอิทธิพลต่อการตัดสินใจมีความหลากหลาย ทั้งในระดับบุคคล ครอบครัว และระบบบริการสุขภาพ อย่างไรก็ตาม ปัจจุบันการสังเคราะห์องค์ความรู้ในเรื่องดังกล่าว ยังมีจำกัด การศึกษานี้ มีวัตถุประสงค์เพื่อทบทวนและสังเคราะห์องค์ความรู้เกี่ยวกับปัจจัยที่มีผลต่อการตัดสินใจเลือกวิธีบำบัดทดแทนไตในผู้ป่วยโรคไตเรื้อรังระยะที่ 4 และ 5 โดยใช้วิธีการทบทวนวรรณกรรมอย่างเป็นระบบ แบบผสมผสาน มีการสืบค้นงานวิจัยเชิงคุณภาพและเชิงปริมาณที่เผยแพร่ ระหว่าง พ.ศ. 2557–2567 อย่างเป็นระบบจากฐานข้อมูล CINAHL, Cochrane, ProQuest, PubMed, Scopus และ ThaiJo พบบทความทั้งหมด 8,411 เรื่อง และหลังจากคัดกรองแล้วเหลืองานวิจัยที่คัดเลือก 14 เรื่อง ประกอบด้วย งานวิจัยเชิงคุณภาพ 11 เรื่อง งานวิจัยกึ่งทดลอง 1 เรื่อง และงานวิจัยเชิงปริมาณแบบพรรณนา 2 เรื่อง ดำเนินการประเมินคุณภาพงานวิจัย ด้วยเครื่องมือ mixed methods appraisal tool (MMAT), Version 2018 พบว่า ทั้ง 14 เรื่องผ่านเกณฑ์คุณภาพที่กำหนดไว้และแยก การวิเคราะห์ข้อมูลเชิงปริมาณและคุณภาพก่อนหลัง จากนั้นนำผล การวิเคราะห์ทั้งสองชุดข้อมูลมาบูรณาการร่วมกันเพื่อตอบคำถามการวิจัย
ผลการศึกษา พบว่า ปัจจัยที่เกี่ยวข้องกับการตัดสินใจสามารถจำแนกออกเป็น 4 ด้าน ได้แก่ (1) ปัจจัย ด้านบุคคล เช่น อายุ โรคร่วม ความเชื่อ และค่านิยมส่วนบุคคล (2) การเข้าถึงและความเข้าใจข้อมูลสุขภาพ เช่น ความซับซ้อนของข้อมูลหรือข้อจำกัดด้านภาษา (3) การสนับสนุนจากครอบครัวและบุคลากรทางการแพทย์ ซึ่งมีบทบาทในการให้ข้อมูลและความมั่นใจต่อการตัดสินใจ และ (4) ระบบบริการสุขภาพและนโยบาย เช่น ความพร้อมของสถานพยาบาล และสิทธิประโยชน์ของผู้ป่วย
ข้อค้นพบสะท้อนว่า กระบวนการตัดสินใจควรได้รับการสนับสนุนจากระบบสุขภาพแบบองค์รวม ที่คำนึงถึงความเชื่อ ค่านิยมทางวัฒนธรรม และความท้าทายที่เกี่ยวข้องกับความซับซ้อนของข้อมูลหรือข้อจำกัดด้านภาษา นอกจากนี้ ยังเน้นย้ำถึงความสำคัญของการมีส่วนร่วมของสมาชิกในครอบครัวและบุคลากรสุขภาพในกระบวนการตัดสินใจร่วมกัน องค์ความรู้ที่ได้สามารถนำไปประยุกต์ใช้ในการพัฒนารูปแบบ การดูแลที่เน้นผู้ป่วยเป็นศูนย์กลางและนโยบายที่ส่งเสริมการตัดสินใจในการรักษาอย่างมีข้อมูล ครอบคลุม และเหมาะสมกับบริบทของแต่ละบุคคล
