การเกิดหลอดเลือดแดงโคโรนารีซ้ายออกจากหลอดเลือดแดงปอดในผู้ป่วยเด็ก: การศึกษาย้อนหลัง 20 ปี ณ ศูนย์หัวใจสิริกิติ์ มหาวิทยาลัยขอนแก่น ประเทศไทย

ภาวะหลอดเลือดแดงโคโรนารีซ้ายออกจากหลอดเลือดแดงปอด (Anomalous Left Coronary Artery from the Pulmonary Artery: ALCAPA) เป็นความผิดปกติแต่กำเนิดของหลอดเลือดโคโรนารี ที่พบ ได้ไม่บ่อยในผู้ป่วยเด็ก ภาวะนี้จะพบได้น้อยแต่เป็นภาวะที่คุกคามต่อชีวิต และอาจส่งผลให้เกิดภาวะการ ทำงานของหัวใจห้องล่างซ้ายบกพร่องและภาวะลิ้นหัวใจไมทรัลรั่ว หากไม่ได้รับการรักษาอย่างทันท่วงทีมักเสียชีวิตตั้งแต่วัยทารก การวิจัยนี้มีวัตถุประสงค์เพื่อทบทวนและวิเคราะห์ข้อมูลย้อนหลังจากเวชระเบียนของผู้ป่วยเด็ก จำนวน 15 ราย ที่ได้รับการวินิจฉัยว่าเป็น ALCAPA และได้รับการผ่าตัดแก้ไข ณ ศูนย์หัวใจสิริกิติ์ภาคตะวันออกเฉียงเหนือ ระหว่างเดือนมกราคม พ.ศ. 2547 ถึงเดือนกันยายน พ.ศ. 2567
ผลการวิจัย จากการบันทึกผู้ป่วย 15 ราย ได้เข้าการวินิจฉัยเมื่ออายุในช่วง 2-192 เดือน (มีค่ามัธยฐาน ที่ 7 เดือน) ส่วนใหญ่เป็นเพศหญิง (ร้อยละ 66.7) อาการที่พบมากที่สุดคือ ลิ้นหัวใจไมทรัลรั่วตั้งแต่กำเนิด (ร้อยละ 100) และ ภาวะหัวใจล้มเหลว (ร้อยละ 73.3) การตรวจคลื่นไฟฟ้าหัวใจพบภาวะหัวใจห้องล่างซ้ายโต ร้อยละ 93.8 และภาวะกล้ามเนื้อหัวใจตายด้านหน้าและด้านข้าง (anterolateral MI) ร้อยละ
73.3 การผ่าตัดส่วนใหญ่เป็นการย้ายหลอดเลือดแดงโคโรนารีโดยตรง ( ร้อยละ 93.3) และ มีการผ่าตัดซ่อมแซมลิ้นหัวใจไมทรัลร่วมด้วย (ร้อยละ 33.3) มีอัตราการเสียชีวิตในระยะแรกหลังผ่าตัด ร้อยละ 33.3 โดยสัมพันธ์กับภาวะหัวใจห้องล่างซ้ายทำงานผิดปกติอย่างรุนแรงและมีลิ้นหัวใจไมทรัลรั่วหลังการรักษาผู้ที่รอดชีวิต พบว่ามีการทำงานของหัวใจดีขึ้นอย่างชัดเจน ร้อยละ 67.1 ผลการติดตามในระยะเฉลี่ย 10 ปี พบว่าผู้ป่วย 10 ราย มีอาการดีขึ้น อยู่ในระดับ New York Heart Association (NYHA) Class I และ มีเพียง
1 ราย ที่อยู่ในระดับ class II
สรุปได้ว่าผลการผ่าตัดรักษา ALCAPA ในระยะแรกช่วยเพิ่มอัตราการรอดชีวิตในระยะยาว อย่างไรก็ดีหากผู้ป่วยมีภาวะหัวใจห้องล่างซ้ายทำงานผิดปกติรุนแรงและลิ้นหัวใจไมทรัลรั่วมากก่อนผ่าตัดจะมีความเสี่ยงต่อการเสียชีวิตจะสูงขึ้น
