The effects of a breastfeeding promotion program on perceived self-efficacy, maternal breastfeeding behaviors, and neonatal physiological jaundice: A feasibility of a randomized controlled trial

This study investigated the feasibility of a randomized controlled trial to examine the effects of a breastfeeding promotion program on perceived breastfeeding self-efficacy, breastfeeding behaviors of postpartum mothers, and neonatal physiological jaundice. Thirty primiparous pregnant women receiving antenatal and delivery care at Bangplee Hospital, Samut Prakan Province, Thailand. They were randomly assigned to either an intervention group (n = 15) or a control group (n = 15).
The experimental group received a breastfeeding promotion program based on Bandura’s self-efficacy theory, consisting of a breastfeeding support video, a leaflet on preventing physiological neonatal jaundice, breastfeeding skills training, and persuasive communication. The program was verified by seven experts, with a content validity index (CVI) of 0.95. Data collection instruments included a questionnaire on perceived breastfeeding efficiencies (CVI 0.84; reliability: α 0.92) and a breastfeeding behavior assessment (CVI 0.92; α 0.83). The experimental group received this program twice during pregnancy. Perceived breastfeeding self-efficacy was evaluated on postpartum period day 3 postpartum; while breastfeeding behaviors were assessed daily on postpartum days 1, 2, and 3 postpartum. The physiological jaundice was diagnosed by a pediatrician on days 2 and 7 of age. Data were analyzed using descriptive statistics, Chi-square, Yates’ correction, and Mann-Whitney U tests.
Results showed significantly higher perceived breastfeeding self-efficacy scores in the intervention group (Mann-Whitney U 16.50, p < .01), along with higher breastfeeding behavior scores day 1, 2 and 3 (Mann-Whitney U 57.50, 41.50, and 54.50 respectively, all p<.01). Neonates in the intervention group had lower rates of physiological jaundice (Yates’ correction 2.981, df = 28, p<.05). These findings suggested that the program is feasible, effective in enhancing maternal perceived breastfeeding self-efficacy, promoting breastfeeding behaviors, and reducing prevent the incidence of physiological neonatal jaundice.
