ประเด็นขัดแย้งทางจริยธรรมและการตัดสินใจเชิงจริยธรรมในการผดุงครรภ์ มีปัจจัยที่เกี่ยวข้องหลายประการ และตามยุคสมัยที่เปลี่ยนแปลงไป บทความนี้ขอนำเสนอประเด็นขัดแย้งทางจริยธรรมและการตัดสินใจเชิงจริยธรรมในการผดุงครรภ์ยุคประเทศไทย 4.0 ที่พบบ่อย ขั้นตอนการตัดสินใจเชิงจริยธรรม และแนวทางการตัดสินใจเชิงจริยธรรมในประเด็นต่าง ๆ สำหรับผดุงครรภ์ ประกอบด้วย 1) การแสดงความยินยอมโดยได้รับการบอกกล่าวของหญิงตั้งครรภ์วัยรุ่น 2) การทำแท้งหรือยุติการตั้งครรภ์ในทารกกลุ่มดาวน์ 3) การรักษาความลับของสตรีตั้งครรภ์ที่ติดเชื้อเอชไอวี 4) การบอกความจริงและการแจ้งข่าวร้ายเมื่อทารกตายคลอดหรือพิการ 5) การปกป้องทารกแรกเกิดในการวางแผนจำหน่ายทารกกลับบ้าน ดังนั้นพยาบาลผดุงครรภ์ควรมีกระบวนการและแนวทางการตัดสินใจเชิงจริยธรรมที่เหมาะสม เพื่อการดูแลมารดา ทารกที่มีประสิทธิภาพและลดความขัดแย้งทางจริยธรรมในการปฏิบัติการผดุงครรภ์
